Job summary
We wish to recruit a full
time Senior Mental Health Practitioner to join our practice in
Deptford, Lewisham.
We are a 15,000
patient training practice based in the Waldron Health Centre, a modern,
spacious facility in the heart of Deptford, North Lewisham. Deptford is a
dynamic, diverse, culturally rich and rapidly changing neighbourhood with
pockets of deprivation alongside new developments. We are situated directly
opposite New Cross station and within walking distance of Deptford DLR,
Deptford station and New Cross Gate station.

a training practice and have a diverse clinical team including Advanced Nurse
Practitioners and an internal Clinical Pharmacist. We also teach medical

Main duties of the job
We have large numbers of students in our population and we have a close link with Goldsmiths University. We are committed to healthcare for all and to ensure everyone benefits we make special efforts to reach out to those patients who society disadvantages. We run a dedicated clinic for the homeless, for asylum seekers and a service for drugs and alcohol addiction. Some of our salaried doctors are involved in research and community advocacy to improve access to health care for marginalised groups.

You will be part of the duty teamworking
alongside our advanced nurse practitioners. You will also work closely with a
clinical team with formal and informal opportunities to discuss patients with
them. You will have an overview of an area of clinical responsibility and you
will be given protected time to devote to this work.

the PCN and we have a current VTS programme director among us. We encourage our

Job Summary:
The Senior Mental Health Practitioner will act autonomously within their professional scope of practice providing care for patients face to face, telephone consultations and home visits.
The post-holder will operate within their own sphere of competence and agreed acceptable limits of practice to provide expert professional clinical advice to patients, carers and colleagues, ensuring clinical safety and excellence.
The Senior Mental Health Practitioner will need to prioritise and triage the needs of patients accordingly, making any necessary referrals for investigations in the appropriate manner.
The post-holder will use skills, knowledge and competencies as a Senior Mental Health Practitioner in order to be responsible and accountable for managing patient caseloads for treatments, referral, admission and discharging patients as appropriate.
Job responsibilities:
- To provide medicine management education, adherence advice and lifestyle advice for patients and their families and/or carers, presenting with mental health needs
- To work with patients and team members to support engagement with appropriate services and treatment which may include onward referral to secondary mental health services or signposting to other mental health services, social prescribers, third sector partners, voluntary services, wellbeing hubs and community-based support groups.
- To support patients in their referral from primary care to secondary care and ensure effective and safe transition.
- To support patients to stay well within their local communities, including psychological support and the re-enforcement of psychology techniques.
- To promote the safety and the wellbeing of patients and their families and/or carers
- To develop a close relationship with the rest of the team, attending practice meetings and having regular informal contact.
- To support GPs in assessing risk by providing expert advice and consultation.
- To support patients on the SMI register to access annual physical health checks
- Refer and signpost service users to most appropriate service for their needs mental health, physical health, social needs
- Apply non-medical prescribing training and expertise to prescribe appropriate pharmacological treatments and conduct routine medication reviews.
- Keep up to date with continuous professional development (CPD) as outlined in NMC code of practice
- Adhere to code of conduct and standards of proficiency as outlined by regulatory bodies
- Conduct clinical audit, routine outcome monitoring and reporting to inform evidence-based practice
