Job responsibilities
Job Summary
To provide support to patients to take pro-active steps
to improve the way they manage their physical and mental health conditions,
based on what matters to them. They support people to develop their knowledge,
skills and confidence or to build their patient activation - in managing
their health and care, to improve their health outcomes and quality of life.
Primary responsibilities
Provide health
coaching support for people with one or more long-term conditions, with the aim
of: improving peoples levels of activation, empowering people to manage
their own health and improve their health outcomes.
Manage and
prioritise a caseload, in accordance with the needs, priorities and support
required by individuals in the caseload.
Work as part of
a multidisciplinary multi-agency team to promote health coaching and to be
ambassadors for Personalised Care and Supported Self- Management, modelling the
coaching approach in their work.
Ensure that
GPs, practice nurses, practice pharmacists and other members of the primary
care team understand the health coach role, how to refer to them, and which
patients may particularly benefit from health coaching.
Support local
health, social care and voluntary sector professionals to make appropriate
referrals to the service.
Promote and
raise awareness of the health coaching service particularly to groups and
communities that experience barriers to access.
Attend and
contribute to team, practice, and PCN meetings and events as required by the
service.
Work flexibly,
adapting to the needs of the service and client group while maintaining the
integrity of the role.
Participate in
regular health coaching supervision and continual learning.
Contribute to
and take part in health coaching workshops in order to spread health coaching
skills to support a mind-set shift among staff in how they have conversations,
integrating a health coaching approach into how they work with people and each
other, and to strengthen a shift towards an enabling a compassionate and
coaching culture in their organisation.
Collect service
user experience and impact of health coaching as part of the delivery of
personalised care. Participate and collect information that measures the impact
of health coaching as an intervention that supports embedding personalised care
into local health systems e.g. collect data entry relating to the health
coaching activity in GP, Local Authority, and hospital clinical systems or
other systems, as required.]
Organisational Position
The
GPA will report to the Practice Manager (Line Manager) and the GP Lead at their
practice.
Individual Responsibilities
Adhere
to DHC and GP Practices policies and procedures and any other relevant
legislation.
Participate
in regular supervision sessions and appraisals with your line manager. Attend
training and development activities as identified and participate in meetings
as required.
Maintain
administration systems and workspace used in a clean and tidy condition.
Maintain
a good working knowledge of Health and Safety procedures and fire precautions
and operate the correct procedures and participate in policy development and
data collection where appropriate.
Work
flexibly to meet the needs of patient
Ensure
mandatory training is up to date at all times.
Contribute
to ongoing/new projects as required.
To
undertake any other duties appropriate to the grade and purpose of the job as
may be agreed by the post holder.
Work Setting And Review
The
post holder will work autonomously to an agreed set of targets and objectives.
They
will manage their own time and case load, providing regular updates/progress
reports to their line manager and to the GP practices they are based in.
Participate
in DHCs appraisal and review system.
Confidentiality
In
the course of seeking treatment, patients entrust us with, or allow us to
gather, sensitive information in relation to their health and other matters.
They do so in confidence and have the right to expect that staff will respect
their privacy and act appropriately.
In
the performance of the duties outlined in this Job Description, the post-holder
may have access to confidential information relating to patients and their
carers, practice staff and other healthcare workers. They may also have access
to information relating to the Practice as a business organisation. All such
information from any source is to be regarded as strictly confidential.
Information
relating to patients, carers, colleagues, other healthcare workers or the
business of the Practice may only be divulged to authorised persons in
accordance with the Practice policies and procedures relating to
confidentiality and the protection of personal and sensitive data.

holder must be willing to attend health coaching workshops and training.
Participation
in an annual individual performance review, including taking responsibility for
maintaining a record of own personal and/or professional development.
Taking
responsibility for own development, learning and performance and demonstrating
skills and activities to others who are undertaking similar work.
Attend
all relevant annual updates.
Inform
the Line Manager of any concerns regarding the Health and Wellbeing Coach role
and any professional development needed.
Be
aware of own professional boundaries and what to do when you have reached them.

The post-holder will
