Job summary

Poole Central Primary Care Network are pleased to offer an exciting opportunity for an experienced, enthusiastic and highly motivated Respiratory Nurse to join the team.

The main aim of the role will be to provide high-quality person-centred care for patients with known or suspected respiratory disease, either within their own homes or in a PCN practice-based clinic, ensuring a cohesive, holistic and evidence-based approach to the management of respiratory care. This will include undertaking annual reviews, post discharge reviews and interim reviews following an acute exacerbation to reduce the risk of readmission.

This role is offered on a part-time basis of 12 hours per week. 4 hours to be worked in clinic on a Friday morning and the remaining 8 hours to be worked on either a Wednesday, Thursday or Friday afternoon (to be discussed at interview)

Main duties of the job

The post holder will be an active member of the PCN Respiratory Team to assist in the delivery of the respiratory diagnostic service, incorporating clinical assessment, spirometry and FeNO testing and supporting diagnosis of respiratory disease for the local PCN populations. The role will also include working closely with the wider PCN Enhanced Care Team, to review patients in their own homes who require an annual respiratory review or have been referred from wider PCN or practice teams.

About us

Poole Central PCN is the second largest PCN in Dorset and one of the first to develop an operational Hub with a central co-ordination team and clinical teams co-located in a dedicated building.

The PCN services comprise the Enhanced Care Team, Care Home Team and Acute Home Visiting working in the community and a range of services focusing on Long Term Conditions, Clinical Pharmacy, MSK, Dietetics, Mental Health and Self-Management.

We work collaboratively with partners in health and social care and are currently developing a number of pathways that involve models of integrated working and information systems, and digital technology have a key part to play in achieving greater efficiency in how we work.

Job responsibilities

To undertake holistic and patient centred respiratory reviews, utilising competency-based skills to provide a full assessment of the physical and psycho-social care needs of housebound/frail respiratory patients, involving carers and relatives.

Within this holistic assessment, to establish an individual's functional capabilities with regards to their respiratory condition, as well as the ability to manage other co-morbidities.

In conjunction with the individual, to develop personalised, evidence-based treatment plans which support the long term management of their condition. This will include supporting compliance and concordance, and adopt health promotion strategies that promote healthy lifestyles, and apply principles of self-care.

Woking closely with other members of the Respiratory Team to facilitate timely diagnostics and treatment plans for the PCN population. This will involve undertaking and interpretation of diagnostic tests, including spirometry and Feno within the Respiratory Hub setting.

To review respiratory medication for therapeutic effectiveness, appropriate to patient needs and in accordance with evidence-based practice and national and local protocols, and within scope of practice.

To provide cognitive assessment and identification of mental health needs, liaising with other multi-disciplinary colleagues as appropriate.

Where required, work closely with other health and social care providers to provide community-based care, and support the needs of the individual.

Following development of a patient focused care plan, to discuss assessment outcomes with patients, carers, their designated GPs and other health and social care professionals, as required.

To identify social isolation and loneliness, being proactive in sign-posting to relevant resources to empower patients to remain active and engaged within their communities.

Using a high level of communication and interpersonal skills, establish effective working relationships with patients, their families and carers.

To recognise and identify a deterioration in an individual's health and act promptly to reduce risk of rapid deterioration or where appropriate avoid hospital admission. Refer onto relevant health professional as required.

To educate individuals and carers/relatives to identify early warnings of deterioration in order to facilitate rapid management of complication or crises.

To facilitate early discharge, where possible, from hospital for case managed patients by co-ordination of care and services to be delivered within primary care/community.

To identify those individuals with more complex health needs and refer for a holistic, multi-dimensional, interdisciplinary assessment with other members of the Enhanced Care Team or MDT specialising in older peoples health and/or specialising in long term conditions.
