This role will be instrumental in bringing together organisations from across the Fylde Coast, children, young people and their parents/carers to implement improvements in mental health provision.
The post is initially offered for 12 months as we start phase 1 of our project. However for the right candidate, who demonstrates a strong work ethic, there may be an opportunity to extend the contract.

Main duties of the job
The
CYP Project Support Co-ordinator will build relationships across schools and
colleges, health care, social care and the voluntary sector, enabling us to
improve mental health services for children and young people in the area. They
will help to create new opportunities for collaboration, for example setting up
regular meetings where representatives can come together to discuss ways to
improve mental health services. They
will seek feedback from children, young people and their carers; work closely
with education and voluntary sector teams and collate resources into easily
accessible formats for our young people.
The
postholder will have the opportunity to learn from very experienced Project
Managers, Mental Health Practitioners and GPs to deliver a truly innovative
project that will make a positive difference to young people.
The
post is initially advertised for 12 months, but there is an opportunity to
extend this if the postholder demonstrates a strong work ethic, collaborative
working and an ability to deliver the required outcomes. The post will be based in Torentum PCN, but
will undertake work that benefits all PCNs in Fylde and Wyre.
About us
The postholder will work with the Fylde and Wyre Partnership Project Manager, who is also the PCN Manager at Torentum PCN. They will be supported by a very experienced GP and Mental Health Practitioner. The postholder will work across the Fylde Coast with colleagues from education, health, social care, the voluntary sector and children and young people themselves.

Set
up and participate in children and young peoples and parent/carer focus groups
across Fylde and Wyre.
Actively
listen to the views of staff working in schools and colleges to identify how
staff in education and health could work more closely together for the benefit
of our children and young people.
Act
as a key point of contact for schools and colleges, keeping them updated on
appropriate resources and referral pathways created during the project.
Work
closely with organisations across Fylde and Wyre to identify resources for
children and young people that support the development of good mental health.
Collate
mental health resources and display them in a format that is easily accessible
and understandable for our young people and their carers.
Work
closely with Social Prescribers, Healthwatch and Befriending Co-ordinators in
our Local Councils to identify and map all of the available services, voluntary
groups and clubs/societies that could be beneficial to our children and young
people.
Collate
feedback from GP Practices, schools and colleges to identify specific examples of
where our children and young people have been waiting too long for mental
health services (or have been declined by services); taking these to our
collaborative action learning sets designed to improve the way our service
work.
Organise
a range of events where key stakeholders will collaborate e.g. setting up
venues, taking notes, producing resources.
Complete
surveys before and after the project to identify changes in satisfaction levels
and record a range of key performance indicators.
Provide
project support e.g. tracking project actions, supporting multi-disciplinary team
meetings (where staff from health, education and schools meet to discuss how
mental health support is best provided) etc.
Lead
on CYP health and wellbeing promotion & campaigns across F&W
