primary care team to build a fantastic mental health service for our patients? The Torentum PCN team (supporting 3 practices
in Thornton Cleveleys) includes Social Prescribers, Care Coordinators, a Medicines
Team, Paramedics and a Mental Health team. This vacancy is for our band 6 Mental Health Practitioner (MHP).
Torentum is known as an innovative and forward-thinking
Primary Care Network (PCN), who supports their staff to develop and grow. We
are excited to extend our existing service to enhance support for our patients. If you join our team you will be supported in your
development, linked in with all the networks and services you will need and be fully supported by our band 7 MHP, PCN Clinical Lead and PCN Clinical
Director.
Your role will be based in Thornton Cleveleys where you will
undertake your role in our 3 practices, during primary care
opening hours - there is no on-call or weekend working required. We encourage potential candidates to come for

The role works entirely within primary care, but the employment contract is held by LSCFT - the Lancashire Mental Health Trust.
Salary will depend upon experience - £35,392 - £42,618

Main duties of the job
The
postholder will be a confident and experienced Mental Health Practitioner (MHP)
who will work as part of a team of PCN Mental Health Practitioners covering 3
busy GP practices in Thornton Cleveleys. The role will spend their time predominantly completing patient assessments, liaising
with primary care colleagues, referring to secondary care as appropriate etc. They
will provide advice and guidance to patients who do not fulfil the criteria for
secondary care, support those awaiting secondary care support, provide physical
and mental health checks for those with mental health needs and support the
band 7 in the triage and management of appointments.
The Mental Health
Practitioner (MHP) will be employed by the Primary Care Network (PCN) and be a
key member of the Mental Health Multidisciplinary Team (MDT) working with
individuals registered with GP practices within Torentum PCN who are presenting
with mild, moderate and stable severe mental health needs. The role will provide
support to patients presenting with the following conditions whose presentation
does not currently meet the criteria for secondary care:

The
Torentum Primary Care Network was formed in June 2020, as a collaboration of 3
GP Practices, Cleveleys Group Practice, The Crescent Surgery and The Thornton
Practice serving a population of registered patients on our list equating to
approximately 33,500 patients. We are known as a supportive, collaborative and
innovative PCN, where all staff work together for the benefit of our patients.
