PCN Pharmacy Technician

Job summary

Would you like to work in a supportive and innovative primary care team to build a fantastic medicines service for our patients? The Torentum PCN team (supporting 3 practices in Thornton Cleveleys) includes 3 Social Prescribers, 3 Care Coordinators, 3 Nurse Associates, a Mental Health Team, Paramedics and a medicines team including 3 Pharmacists and a Pharmacy Technician. The PCN are looking to recruit 2 additional Pharmacy Technicians to increase the number of technicians in the team to 3.

Your role will be based in Thornton Cleveleys where you will undertake your role in our 3 practices during primary care opening hours there is no on-call or weekend working required. Main duties of the job

Duties & Responsibilities:

Carry out medicines optimisation tasks including effective medicine administration, medication reviews, and medicines reconciliation.

Support medication reviews and medicines reconciliation for new care home patients and synchronising medicines for patient transfers between care settings and linking with local community pharmacists.

Work under supervision to ensure the safe, accurate and timely supply of prescribed medication to patients.

Provide technical and administrative support to the pharmacy team.

Take a central role in the clinical aspects of shared care protocols and liaise with specialist pharmacists for more complex patients.

Assist in the delivery of medicines optimisation and management incentive schemes and patient safety audits.

Support the implementation of national prescribing policies and guidance within GP practices, care homes and other primary care settings.

About us

The Torentum Primary Care Network was formed in June 2020. It includes 3 GP Practices, The Cleveleys Group Practice, The Crescent Surgery and The Thornton Practice, serving a population of approximately 33,500 registered patients. The PCN also employs twenty staff, working across all 3 of the practices.
