Greater Middlesbrough Primary Care Network (PCN) is seeking a Clinical Care
Coordinator to join our growing team.
The successful candidate will be
part of a dynamic and forward-thinking PCN led Multi-Disciplinary Team (MDT).
Clinical leadership will be provided by the PCN Clinical Directors and Clinical
Leads.
The successful candidate will be
based in a local cluster of General Practices as part of the PCN.
We are looking for individuals
who will lead clinical care home rounds
& coordinate the work of
healthcare professionals and non-clinical staff in the care of patients
registered at GP practices within the wider PCN population. The role includes
responsibility to work with the PCN Operational Manager and the practices to
support and ensure compliance with the reporting requirements of the PCN
Directed Enhanced Services (DES).

Salary is negotiable depending
on experience and qualifications up to Band 7 AFC.
Main duties of the job
The role will involve working
across organisational & professional boundaries to provide clinical care
and leadership and co-ordinate the treatment, care, management, and empowerment
of care home residents.
The individual will take part in
the weekly care home rounds to include
- Lead
weekly ward rounds in our aligned Care Homes
- Weekly MDT's with our Learning
Disability EHiCH colleagues in our LD Care Homes
- Review medication
The Clinical Care Coordinator
will work closely with the Care Coordinators, GPs and practice teams to manage
a caseload of patients, acting as a central point of contact to ensure
appropriate support is made available to them and their carers; supporting them
to understand and manage their condition and ensuring their changing needs are
addressed.

Network (PCN). The Network supports over 70,000 patients from 8 practices
across the area.

closely work with our local voluntary organisations and other partners to
develop new models of care to benefit our patients.
The role will provide additional
flexibility to the PCN by adding to the growing expansion of the primary care
workforce.
