Job summary
The post holder will work with the wider Greater Middlesbrough Primary Care Network (PCN) team
to ensure that all aspects of the Primary Care Network Contract Directed
Enhanced Service and its associated service specifications, relevant to this
post, are realised.
The post holder will take responsibility for areas of chronic disease
management within practices and undertake clinical medication reviews to
proactively manage patients with complex polypharmacy.
The post holder will provide primary support to general practice staff
with regards to prescription and medication queries and help support the repeat
prescription system. They will deal with acute prescription requests and
medicines reconciliation on transfer of care for patients. They will support
the development and delivery of systems for safer prescribing, providing
expertise in clinical medicines advice whilst addressing both public and social
care needs of patients in primary care.
The post holder will provide clinical leadership on medicines
optimisation and quality improvement and manage some aspects of the Quality and
Outcomes Framework and Investment and Impact Fund where appropriate.
The role is pivotal to improving quality of care for patients and
operational efficiencies within primary care and therefore requires motivation
and passion to deliver excellent service within general practice.
Main duties of the job
Responsibilities may include but not be limited to the following:
See patients with LTC(s) where medicines optimisation is required and
review the on-going need for each medicine, making recommendations to GPs for
medicine improvement where appropriate
Undertake clinical medication reviews with patients and make
recommendations on prescribing and monitoring
Work with care home staff to improve safety of medicines ordering and
administration
Attend and refer patients to multidisciplinary team meetings
Where appropriate and required, provide patient facing clinics and / or
phone consultations for those with questions and concerns about their medicines
in the practice
Reconcile medicines following discharge from hospitals, intermediate
care and into care homes
Manage caseload of patients with common/self-limiting/minor ailments
while working within a scope of practice and limits of competence – signposting
to community pharmacy and referring to GPs or other healthcare professionals
where appropriate
Set up and manage systems to ensure
continuity of medicines supply to high-risk groups of patients identification
of cohorts of patients at high risk of harm from medicines through pre-prepared
practice computer searches
