Job summary

We are sadly seeing our Health Care Assistant retire at the end of March this year and as a result, we will have a vacancy for an enthusiastic, flexible Health Care Assistant to support the practice team. Ideally experience of phlebotomy, a Care certificate and able to start ASAP is preferred and training will be given to the right candidate.

We are a respected, patient centered, GP Practice with a good CQC rating.

Our clinical team comprises of 4 GP's, 2 ACP's Nurse prescribers, Paramedic, Nurses, HCA's and Clinical Pharmacists and we are looking for an additional HCA to join our forward thinking team.

Job responsibilities

JOB TITLE: Health Care Assistant

ACCOUNTABLE TO: Practice Manager

SUPERVISED BY: ACP Lead

HOURS OF WORK: Part Time up to 24 hours per week,

START DATE: ASAP

SALARY: Depending on experience

PROBATION PERIOD: SIX months

The Practice Philosophy is:

AIMS OF THE POST:

To be an actively participating member of the Primary Health Care Team, caring for practice patients including treatment, preventative care, screening, and patient education ensuring patient confidentiality in all matters.

Main duties and responsibilities of the post:

Care and assessment of practice patients

To provide support to the practice nursing team to enable them to function effectively.

To provide care to patients according to and by following practice approved protocols in long term condition reviews

To maintain clinical stocks such as vaccines and drugs as delegated

Specific areas of responsibility

To take and record blood pressures using templates as requested by the doctors or nurses.

With other HCAs maintaining the home monitor blood pressure register.

To perform LTC reviews and record on clinical templates

To perform venepuncture for patients as required

To test urine specimens using appropriate equipment and record as requested

To perform other clinical tasks as trained and agreed to meet the needs of the practice. These include suture removal, ear irrigation, wound care (if completed the relevant courses), B12, NHS health checks, Chronic Disease Management checks and pre diabetic clinic checks.

To perform any other task within the skills of the post holder.

To administer Flu and Covid Vaccines to patients aged 18 plus.

Administration and professional responsibilities

Participate in the administrative and professional responsibilities of the practice team.

Ensure accurate completion of all necessary documentation associated with patient health care, registration and claims, including all necessary computer entries.

Ensure collection and maintenance of statistical information.

Attend and participate in HCA meetings. Take an active role in the future development of the integrated nursing team.

In conjunction with other team members take part in audit of work for effectiveness and future planning.
