Job summary
St
Helens Central PCN are looking to recruit a Clinical Care Coordinator to
work within our Frailty Team within St Helens Central Primary Care Network.
This
is an exciting role where you will be using your experience and knowledge to support and complement our existing team providing and monitoring patient centred care, across the patient population of St Helens Central network.
The
post holder will be based within the Frailty Team covering the Practices within the network. You will be required to travel from base to
care homes/patient homes and to other venues during fulfilment of their duties,
therefore a full clean driving license and access to a reliable vehicle insured
for business use is essential.
You
will be motivated, proactive, and committed with excellent
communication skills. You will work
independently and within the team under the leadership of our ANP, supporting with all aspects of and improving
on the safety and quality of patient care at every opportunity.
The role will involve working within core hours 8am - 6:30pm however a degree of
flexibility is required.
We welcome applications from those who have previous Primary Care experience, with access to a car with a full clean
driving licence.
Main duties of the job
The role of the Clinical Care Co-ordinator is to support the
Frailty Team and Practice staff to identify and support people to reduce the
risk of unplanned hospital admissions and to effectively support those
individuals in the community. To pro-actively engage with people deemed to be
at risk of hospital admission or health deterioration. To pro-actively engage
with people living in care home settings and to be the key contact within the
Frailty team.

Medical Centre, Lingholme Health Centre, Marshalls Cross Medical Centre,
Newholme Surgery, Ormskirk House Surgery, Parkfield Surgery and Phoenix Medical
Centre, serving a population of approximately 36,000 patients.
St
Helens Central PCN is a proactive, collaborative team who are very well
supported by their Clinical Director and the staff in the 8 practices. There is

Care.
