Main duties of the job

We are looking for someone to become an integral part of the nursing team, they will work with the nurses and other clinical staff and follow the job description.

About us

Are you a passionate HCA looking for a rewarding role in a vibrant community? Wellington Way Health Centre has an amazing opportunity for you to join our caring and motivated Nursing Team of three nurses, one nurse associate and senior healthcare assistant and two phlebotomists.

In the wider team there are four partners, three salaried GP's, two Physician Associates, one pharmacist, a first contact physio plus a Mental Health Practitioner.

We are a teaching practice for Queen Mary University and Kings College, we also have a trainee practice nurse.

We welcome our practice staff to attend clinical meetings twice a month and to get involved in other meetings if required.

Job responsibilities

Job Summary:

The purpose of the role is to assist the GPs and the nursing team in the service and delivery of the care management of the practice population.

Particular Responsibilities:

The post holder works under indirect supervision of the practice nurse team and undertake tasks and duties delegated by the practice nurses and GPs. Work collaboratively with the general practice team to meet the needs of the patients, following policy and procedures while providing supervision to more junior staff.

Key responsibilities

Communication

Communicate effectively with patients and carers, recognising their needs for alternative methods of communication

Undertake effective communication with patients in specific clinical situations-developing good rapport with them

Communicate effectively with other team members

Practice, personal and people development

To work as part of a primary health care team in a collaborative manner

To meet regularly with the practice team to discuss health care assistants role and development.

To participate in practice-based learning appraisal and to compile a personal learning portfolio

To undertake other duties appropriate to the grade required by the practice

take responsibility for own developmental learning and performance, including participating in supervision

Mentoring/support staff in building their confidence with clinical procedures such as: - phlebotomy, use of EMIS templates and infection control.

Take responsibility for maintaining a record of own personal development

Work with the practice to assess appropriate training based on identified need

To recognise and understand the roles and responsibilities of individuals working in the primary health care team

Be aware of the legal issues pertinent to the role of a HCA (Band3)

Identify own work based boundaries.

Practice organisation

To complete returns to external agencies related to the delivery of clinical care in the practice

To gather information from written and computerised records to support clinical audits and health needs assessment

To maintain accurate records of patients care in both manual and electronic formats.

Health, safety and security

Use and encourage others to adopt appropriate infection control procedures and maintain work areas in each clinical room so that they are clean, safe and free from hazards reporting of any potential risks identified, including:

1. hand washing

2. universal hygiene procedures

3. collection and handling of laboratory specimens

4. clinical waste

5. safe handling sharps bin

Clinical Duties health and well-being:

To work within the agreed protocols to:

Undertake, record and follow guidelines for the task for which you have received appropriate training

Urinalysis and preparation of specimens for investigation by the pathology laboratory

Measuring and recording following physiological measurements in routine presentations such as:-

1. Blood pressure/annual check

2. Pulse rate and rhythm

3. Height and weight-body mass index

4. ECG

5. Peak flow readings inhaler technique

6. Cardiovascular disease/annual check

7. Diabetes annual check

8. Diabetes monitoring machine education

9. PHQ9-depression checklist

Support and monitor patients during nebulisation therapy

Prepare and maintain environments and equipment before, during and after patient care interventions

Assist in raising awareness of health and well-being, and how it can be promoted

Give accurate and appropriate information to patients and groups within own competence

Where appropriate, provide support and encouragement to self-manage their conditions

Have a basic understanding of long-term conditions including diagnostic interventions, treatments, and the principles of care and self-management.

Plan, develop and implement health promotion activities as appropriately e.g. smoking cessation, exercise and diet

Assess patients readiness to change and provide motivation support as appropriate in cases of smoking, obesity and healthy living for older people

To administer influenza & pneumococcal vaccine under indirect supervision of the regulated professional

Vitamin B12 injection under indirect supervision of the regulated professional

To gain the underpinning knowledge and practical skills to administer basic life support

Referring patients appropriately to other agencies:- smoking cessation advisory service, Podiatrist, Retinal screening department, pharmacy first-minor ailments scheme and District Nurses.

Assisting with clinics as appropriate.

Acting as chaperone for other clinicians when required

Wound care only after patients have been assessed by a senior nurse

Information processes

Record information and activities undertaken with patients and carers in an accurate and timely fashion using manual or computerised system as appropriate

Inputs accurate Read codes about patients to ensure accurate retrieval of information for monitoring and auditing processes

Maintain confidentiality or information related to patients, relatives, staff and the practice

Take the necessary precautions when transmitting information.

Administration:

Emergency drug check and record keeping

Emergency equipment check and recording, this includes adrenaline in each clinical room, oxygen levels, Defibrillator daily check for green light. Recording and signing that all checks have been carried out.

Ensuring that vaccine refrigerators temperature is checked and recorded twice daily

Ordering vaccines and maintaining stock levels

Pathology stock check and ordering

Any other administrative work to support the practice clinical work

Ensuring the cold chain is maintained when handling vaccines
