Job summary

With the advanceddevelopment of Primary Care Health Networks (PCNs) in Tower Hamlets, PCN 6 Bromley by Bow & Stepney Health (B&SH) CIC are looking for a highly motivated, innovative person to join our General Practice Teams as a Care Coordinator.

Main duties of the job

The role is pivotal in ensuring all patients receive the best possible care and service. The Care Coordinator role will support the Practices within the PCN in coordinating all key activities including access to services, advice and information, and ensuring health and care planning is timely, efficient, and patient-centred. The role will include supporting digital initiatives and includes responsibilities for the co-ordination of our Patients journey with a Long Term Condition through primary care.

Fuelled through the additional funding available via the national DES, it is currently under-going radical expansion into new areas including direct patient care. Now is a very exciting time to get involved in Primary Care.

About us

BASH CIC was formerly known as MEEBBB Health CIC, has been established for 12 years, and employs a number of staff for member practices. In 2010 the then Primary Care Trust, Local Medical Council and GP surgeries in Tower Hamlets set up Networks composed of member practices linked to a Network Incentive Scheme (NIS) as a contract for additional work over and above their standard GP contracts; this has now changed to the LES (Local Enhanced Services). The reason for establishing the PCNs was due to the high levels of deprivation in the borough, and to address the chronic underfunding in primary care.

Job responsibilities

To work closely with our PCN, practices, and the multidisciplinary team to provide a central co-ordinated role in patient care planning and delivery, mostly but not exclusively in the context of the Networks Housebound Programme.

Key responsibilities of the post

  • To co-ordinate care packages for patients and their carers as identified by clinicians across health, social care and mental health services as appropriate.
  • To provide a single point of access for staff & service users.
  • To support the provision of continuity of care and act as a point of contact for families, residents, and professionals for identified patients in your caseload.
  • To actively manage patients care plan delivery and liaise with the PCNs clinicians and Allied Professionals as needed.
  • To help people to manage patients needs through answering queries, making and managing appointments, and ensuring that people have good quality information to help them make choices about their care.
  • To assist people to access self-management information, interventions that support them in their health and wellbeing and increase their activation.
  • To actively support the PCNs activities around immunisation including the delivery of the Covid 19 vaccination programme.
  • To work with the Clinical Lead and support the further development of new services including the Housebound Programme
  • To help support and coordinate monthly, quarterly, annual dashboard data, KPIs and additional target related performance reports.
  • To support the coordination and delivery of MDTs including minute taking.
  • Collate, prepare and submit data for returns to monitoring bodies
  • Interpret data and communicate data outcomes
  • To maintain accurate patients records.
  • Assist with in the production of information for auditing purposes

To undertake any other duties commensurate to the role, particularly supporting new ways of working including remote working.

Attend all relevant and required training and comply with Network arrangements for continuing professional development.

Support PCN Locality work steams

To support intelligent call and recall i.e. Covid-19 booster vaccine, flu jab.

To support Patient Participation Groups

Supporting patients to use surgery pods and other online services

To support patients with long term health conditions

  • Good time management and prioritisation skills.

  • Experience of developing relationships with a wide variety of people.
