Job summary

With the advanced development of Primary Care Health Networks (PCNs) in Tower Hamlets, Bromley by Bow & Stepney Health (BASH) CIC are looking for a highly motivated, innovative person to join our General Practices Teams as a PCN Physician Associate

Main duties of the job

The Physician Associate is a trained healthcare professional who works directly under the supervision of a doctor as part of the medical team. They are usually generalists with board medical knowledge but can develop expertise/specialisms in a field. The responsibilities of the role include direct patient contact through assessment, examination, investigation, diagnosis and treatment. The Physician Associate will have a key role supporting delivery of the new Primary Care Network Contract DES Service specifications.

The successful candidate will ideally have a minimum of 2 5 years of knowledge and experience in the field. The candidate must demonstrate the ability to work collaboratively within a Network of GP Practices, you will be supported by our multidisciplinary team already consisting of Physician Associates.

Personal Professional Development support will be offered by the Primary Care Network.

About us

BASH CIC was formerly known as MEEBBB Health CIC, has been established for 12 years, and employs a number of staff for member practices. In 2010 the then Primary Care Trust, Local Medical Council and GP surgeries in Tower Hamlets set up Networks composed of member practices linked to a Network Incentive Scheme (NIS) as a contract for additional work over and above their standard GP contracts; this has now changed to the LES (Local Enhanced Services). The reason for establishing the PCNs was due to the high levels of deprivation in the borough, and to address the chronic underfunding in primary care.

Job responsibilities

The Physician Associate is a trained healthcare professional who works directly under the supervision of a doctor as part of the medical team. They are usually generalists with broad medical knowledge but can develop expertise/specialisms in a field. The responsibilities of the role include direct patient contact through assessment, examination, investigation, diagnosis and treatment. The Physician Associate will have a key role in supporting delivery of the new Network Contract DES Service specifications.

As part of this new and exciting role you will be working in local GP practices as part of the primary care team, the post holder will provide highly specialist advice for the presenting patient from initial history taking, clinical assessment, diagnosis, treatment and evaluation of care. They will demonstrate safe, clinical decision-making and expert care, including assessment and diagnostic skills, for patients being treated.

The post holder will demonstrate critical thinking in the decision-making process. Under the supervision of a GP Partner and the PCN Clinical Director, they will work collaboratively as part of the primary care team and provide highly specialist advice to meet the needs of patients, and their relatives/carers, while at all times working within the agreed policies and procedures supporting the way care is provided.

Authority to assess and treat will be granted in line with demonstrated skills and competencies. At no time will the post-holder work outside the limits of authority granted. Mentorship and supervision will be provided by a designated GP at all times. The level and type of supervision will be dependent on the post holders possession of skills and knowledge and be determined by those providing mentoring and supervision.

Main duties and responsibilities

CLINICAL

Provide highly specialist advice, treatment and care planning to patients as delegated and agreed by the supervising GP in line with Practices policy and using established clinical guidelines.

Consult with and refer patients to GPs, medical specialists and other health professionals as considered necessary.

Interview patients, take medical histories, perform physical examinations, analyse, diagnose and explain medical problems.

Formulate and adjust as necessary, diagnoses and treatment plans, in consultation with supervising GP where necessary.

Recommend and explain to patients appropriate diagnostic tests and treatment.

Instruct and educate patients in preventative health care.

Confer with patients by telephone as required to provide highly specialist advice.

Order laboratory tests as required and in line with parameters set by the supervising GP.

Fully document all aspects of patient care and complete all required paperwork for legal and administrative purposes.

Perform a variety of research and analysis tasks associated with medical diagnosis and treatment.

Research unusual symptoms and treatment options, through consultation with supervising GP and other medical specialists and by undertaking literature reviews and the like.

Give highly specialist clinical instructions to nursing staff and Primary Care Team members as required.

Assist in clinical instruction and supervision of medical students as required.

Confer regularly with GPs, medical specialists, those working at hospital level and members of the primary care team to plan and co-ordinate activities, exchange information, investigate and resolve problems in relation to the care of individual patients.

Encourage and ensure good working relationships with all members of the Primary Care Team.

Implement the policies and procedures of the practice in which the post-holder is working and propose changes to policies and practices where they professionally believe they require amendment.
