Job summary
The Care Co-ordinator will work closely with GPs and other primary
care professionals to support patient care in a multi-faceted and adaptable
way.
This role will be part of the Twyford Surgery team and will work
alongside a multi-disciplinary team of colleagues to ensure that patients
receive the appropriate support they need.
Working closely with the Operations Manager and Practice Manager, they
will support the effective workload management of GPs and other clinical
colleagues.
The Care Coordinator will support clinicians to bring together a
patient’s identified care and support needs and ensure defined cohorts of
patients have a single personalised care support plan in line with best
practice, based on what matters to the person. They will ensure that
clinicians have access to the tools they need to develop a personalised care
plan.
The Care Coordinator will
help patients and their carers with queries to allow them to navigate the
primary health care system. This will involve knowledge of and signposting to
relevant health services in primary care, as well as providing excellent
quality written or verbal information.
Main duties of the job

referral and liaising with relevant clinicians regarding their care

Acting as a non-clinical cancer “Champion”, coordinating referral
practices, safety netting, and screening activity in conjunction with the
practice cancer lead
·
Act as a point of contact between Allied Health Professionals employed
by the Primary Care Network and the practice, assisting with case finding and
appointment booking and referrals as needed
·
Deliver reception services as needed by the practice, working as a
receptionist for a proportion of worked hours and being knowledgeable about
services delivered in the local health care system
·
Assist administrative colleagues with various tasks as required,
including patient registrations/deductions, online consultations and NHS app
access for patients
·
Coordinate covid vaccination for the practice, booking patients and
performing data entry and reports as needed
·
To co-ordinate clinics, liaising with the schedulers and contacting the
identified patients with appointments
