Job summary
We have a vacancy for an experienced Practice Nurse qualified in all chronic disease management and treatment room nursing.
We operate from two sites in rural Cheshire, Tattenhall and Farndon. Session times and days to be agreed with the successful applicant. Start date would be as soon as possible. This is a permanent position with a highly successful and motivated team of staff to support you.
Main duties of the job
Provide Practice nurse duties for our 8,700 patients. This to include annual reviews, ECG examinations and the full range of Practice Nurse duties on a shared basis with our two other nurses

Job responsibilities
To manage patients with chronic disease, running specific clinic sessions and maintaining robust call and recall systems
Participate in national screening work such as cervical cytology and deliver the national immunisation and vaccination schedule
Treat patients with minor injuries
Provide directly and assist GPs with contraceptive services
Break bad news to patients/parents/carers and provide appropriate ongoing support to meet identified needs
Act as chaperone when required
Undertake health promotion to help educate patients
Select, carry out and interpret results of specific tests
Undertake anticoagulation clinics to include interpretation of blood tests and dosing when using INR Star
Ensure adequate infection control procedures in treatment and consulting rooms
Administration
To maintain accurate patient records and enter onto the computer using agreed Read Codes
Ensure accurate completion of all necessary documentation associated with patient health care
To collect data and participate in regular Practice audit
To contribute to the preparation of any Practice development plans
The checking and recording of medical supplies on receipt
General
To maintain own annual registration with the NMC and adhere to Code of Professional Conduct
To maintain a safe and therapeutic environment for all PHCT staff, patients, carers and co-workers from other agencies. Ensure all statutory regulations (e.g. Health & Safety, COSHH, etc.) are implemented and adhered to
To promote a professional approach at all times by attire and attitude to patients and staff both in and out of the Practice
To maintain own continuous professional development and ensure PDP kept up-to-date in line with the requirements of the Practice
Attend and participate in staff and Practice meetings and in-house clinical training sessions
Any other reasonable duties that may be required from time-to-time
