Job summary
The PCN Pharmacy Care Co-ordinator supports the Primary Care Network Pharmacy Team by coordinating the administrative and operational processes that underpin safe and effective patient care. Working closely with clinical professionals, you will ensure workflows are organised, clinical reviews are scheduled and patient information is accurately maintained. The role helps to deliver structured medication reviews and medicines optimisation, enabling clinicians to focus on patient care while operational systems run efficiently. You will act as a key coordination point across teams and external partners, supporting quality improvement and excellent service delivery within primary care.
The role is for 16 -20 hours per week. Hours and working pattern to be discussed at interview.
Main duties of the job
Provide administrative and clerical support to the PCN Pharmacy Team, coordinating clinical workflows and ensuring patient records are accurate, up to date and confidential. Arrange structured medication reviews, appointments and follow-up actions, including requesting and recording patient information such as blood pressure readings to support clinical decision-making. Manage and prioritise correspondence and workflow, creating tasks and updating clinical systems to ensure timely action and effective communication.
Run reports and gather data to support medicines optimisation, safer prescribing and quality improvement initiatives, including audit and data validation activities. Liaise with community pharmacies regarding prescription queries and supply issues, communicating relevant information to clinicians. Support vaccination programmes, data input and reporting as required, and assist with meeting Network Contract obligations and service objectives.
Maintain accurate documentation and filing systems, ensuring information is accessible and secure. Coordinate meetings, prepare documentation and support the production of reports for clinical and governance purposes. Work collaboratively with the PCN Pharmacy Team and wider network to deliver coordinated care and continuous service improvement.

Role Summary

ensuring patients receive the best possible care and service through
coordinating and supporting the delivery of all key activities of the PCN
Pharmacy Team. This includes the coordination of clinical reviews, reporting
and managing incoming and outgoing correspondence and workflow for both teams
across the week. This role supports multiple clinical professionals including
Clinical Pharmacists and Pharmacy Technicians.
Main Responsibilities & Duties
Provide efficient and comprehensive administrative and
clerical support to the PCN Pharmacy team on a day-to-day basis whilst working
with the team.
Coordinate patient care for the PCN pharmacy team by
requesting and recording blood pressure readings, arranging appointments,
following up on test results, and scheduling subsequent consultations to ensure
timely and effective patient management.
Coordinate and arrange structured medication reviews for
the PCN Pharmacy Team.
Provide appointment rota and template support for the PCN
Pharmacy Team.
Run reports and gather information from a variety of
sources to support the PCN Pharmacy Team with continuous improvement, safer
prescribing and deprescribing where appropriate (polypharmacy).
Co-ordinate medication reviews or changes for the PCN
Pharmacy Team.
Liaise with community pharmacies on the supply of
prescribed drugs, alert our clinicians across the PCN of any supply issues and
forward information on the recommended alternatives.
Respond to patient related queries from colleagues or
teams across the PCN or from patients, carers, secretaries, NHS administrators,
Consultants or the Medicines Management Team in a courteous, timely and
efficient manner, by telephone, letter and email, occasionally face-to-face and
maintain precise records of the communications.
Continuously
monitor, manage, and prioritise incoming and outgoing correspondence and
workflow for the PCN Pharmacy Team initiating responses where appropriate.
Practical application of TPP SystmOne and Microsoft Teams
for coordinating workload, organising individual team members caseloads and
appointments, and developing reporting mechanisms.
Coordinate agendas, prepare patient lists and take minutes
of key meetings
Support the PCN, its member practices and the PCN
Pharmacy Team Lead with aspects of the Network Contract Directed Enhanced
Service (DES) including the Impact and Investment Fund (IIF), Medicines
Optimisation and Structured Medication Review (SMR) areas.
Ensure you are keeping Patient information updated and
that Patient information is secure and kept confidential at all times.
Maintain accurate Patient records promptly and quickly -
entering data in accordance with reporting requirements.
