Summary
This is an exciting opportunity
for a compassionate, dedicated, and experienced nurse to make a real difference
in the lives of PCN Care Home residents, while also providing proactive care to
patients with complex needs. Our Primary Care Network (PCN) is seeking an
Enhanced Nurse to join our friendly, innovative, and forward-thinking team.
You will work closely with the
PCN Manager and Clinical Lead to provide high-quality, patient-centered care
across our care homes. The successful candidate will be someone who truly cares
about making a positive impact on patient care and well-being. You will lead
clinical care home rounds, coordinate the efforts of healthcare professionals
and non-clinical staff, and contribute to the ongoing enhancement of our
services.
This role is pivotal in
supporting compliance with the PCN Directed Enhanced Services (DES) reporting
requirements, enhancing care quality and patient outcomes. If you are
enthusiastic about developing your skills within primary care and delivering
exceptional care to those who need it most, we would love to hear from you.
Full time or part time will be considered.
PCN's core business hours are
Monday through Friday, 0830 - 1630.

The Yeovil Primary Care Network (PCN) is an exciting and creative place

We are a cooperation of five GP practices in Yeovil and a central PCN Hub team

- Leading and conducting regular ward rounds in aligned Care Homes, delivering proactive and patient-focused care.
- Reviewing medication and working effectively within a multidisciplinary team (MDT) to ensure patients receive the best possible care.
- Collaborating with patients, their families, and the MDT to conduct assessments, develop and implement tailored care plans, and evaluate outcomes.
- Using your clinical expertise and evidence-based practice to tackle complex clinical issues.
- Delivering innovative clinical care within evolving care models, staying adaptable and responsive to changing needs and technologies.
