This role requires the successful candidate to have either graduated from a Centre for Advancing Practice accredited MSc advanced practice programme or completed the Centres ePortfolio (supported) Route.
Role Summary
This is an excellent opportunity for an experienced registered practitioner / nurse to help PCN Care Home Residents while also providing proactive care to patients with complex needs.
The successful candidate will be a caring, compassionate, and dedicated experienced registered practitioner / nurse who, within their professional bounds, will provide care for the presenting patient from initial history taking to clinical assessment, diagnosis, treatment, and evaluation of care.
They will make safe, competent clinical decisions and provide professional treatment, including evaluation and diagnostic skills.
The postholder will commit to using critical thinking in the safe clinical decision-making process.
They will interact and collaborate with the PCN and general practice teams to address patients' needs while also supporting policy and procedure implementation.
When not visiting patients in their homes or care homes, the poster holder will work on-site at Abbey Manor Medical Centre in Yeovil.
PCN's core business hours are Monday through Friday, 0830 - 1630.
Main duties of the job
Responsibilities
- Participate in regular MDT meetings and Care Home ward rounds to assess, diagnose, plan, implement, and evaluate treatment/interventions and care for patients.
- Clinically examine and assess patient needs from a physiological and psychological perspective, and plan clinical care accordingly.
- Assess, diagnose, plan, implement, and evaluate interventions/treatments for patients with complex needs.
- Proactively identify, diagnose and manage treatment plans for patients at risk of developing a long-term condition.
- Diagnose and manage both acute and chronic conditions, integrating both drug- and non-drug-based treatment methods into a management plan.
- Prescribe and review medication for therapeutic effectiveness, appropriate to patient needs.
- Support patients and Care Home staff to adopt health promotion strategies that promote healthy lifestyles and apply principles of self-care.
- Assess, identify and refer patients presenting with mental health needs in accordance with the NSF for Mental Health.
- Implement and participate in vaccination and immunisation programmes for both adults and children.
