Job summary
The PCN Lead Nurse will initially work with the Clinical Lead to develop our Chronic Disease clinics and Community Investigation Hub (CIH) and will be key in the development of other nurse led services.
They are accountable for the delivery and standards of care provided to patients, ensuring safe, effective, person-centred care and will be responsible for the supervision of staff working within the CD Clinic and CIH.
Support and supervision for this role will be provided by our newly appointed, innovative and forward thinking Clinical Lead.
Main duties of the job
Initially to develop and lead the PCN Chronic Disease Clinic and Community Investigation Hub, promoting interdisciplinary team and collaborative working practices
To manage and oversee the staff associated with the Chronic disease hub and Community Investigation hub. Working alongside staff from other PCN practices and wider NHS community
Develop, implement and embed health promotion and well-being programmes
Implement and evaluate individual treatment plans for chronic disease patients. Supporting the QoF and IIF PCN targets.
Manage and support patients at risk of developing long term conditions, preventing adverse effects to patients health
Responsible for ensuring the delivery of safe and effective nursing care to the PCN population
Provide highly visible and accessible professional leadership and demonstrate expert knowledge and standards of clinical practice
Act as the PCN Infection Prevention Control (IPC) lead, developing and working to IPC Policy providing training, support and evidence of compliance to the PCN Manager & PCN Clinical Lead
Implement new clinical services after agreement by the PCN Board
Facilitate and promote a learning culture that encourages others to develop their full potential

e.perform investigatory procedures needed by patients and those requested by GPs and the wider PCN
Support the delivery of anticipatory care plans
Communicate proactively and effectively with all colleagues across the multi-disciplinary team, attending and contributing to meetings as required
Collaborate with other members of the PCN including doctors, nurses and other AHPs, accepting referrals and referring to them for specialist care
Maintain accurate and contemporaneous health records appropriate to the consultation, ensuring accurate completion of all necessary documentation associated with patient health care and registration with the practice
Work with patients in order to support compliance with and adherence to prescribed treatments
