The Medicines Hub is an innovative new development and we are looking for a dynamic pharmacist to lead our highly skilled team, you will be part of the team but also expected to work autonomously but never without support .You will be joining us at the beginning of our journey and will be part of developing how we work. The PCN strongly believes in empowering their team and fosters an environment that is conductive to learning. We are committed to developing our staff, if not already qualified you will be enrolled on the Primary Care Pharmacy Education Pathway and supported in qualifying as an Independent prescriber. The achievements of the team are based on collaboration of all team members; the individual skills of each team member are recognised and embraced.
Main duties of the job
Job Summary:
To manage the Medicines Hub pharmacists and technicians and support the hub team. Support the General Practitioners, Specialist Nurses and the wider team by providing expertise, management and leadership in regard to medicines, prescribing, regulatory affairs, audit, compliance and best practice with regards to medicines and Medicines Optimisation.
To work with patients, their carers/relatives, practice staff, primary care teams and secondary care teams to ensure patients receive a service around their medicines that is of high quality, efficiency, cost effectiveness and an overall good experience for the patient. This will also include ensuring patients have the correct access to medicines that meets their needs, their medication is optimised, and they understand and are part of the decision-making process around their medication prescribing.
Through working with the Clinical Director, Practice Manager, Service Delivery Leads. CCG, CSU and colleagues in Secondary Care, to ensure services are cost effective and any targets on cost reduction are met.
To lead and be responsible for aspects of audit, compliance and inspection with regards to any relevant standards from statute, legislation, registration, CQC, MHRA, DOHSC, etc. around medicines and prescribing.
About us
We are building a team of pharmacists, pharmacy technicians, practice medicine managers, prescription clerks and support staff to deliver a medicines optimisation service for Workington town PCN. Workington PCN is a single practice servicing 34,000 patients within Workington and the surrounding area.
The aims of the hub are to:-
1. Improve the quality and safety of care offered to patients
2. Improve patient access to medication queries
3. Provide a medicines management system for the PCN
4. Increase the utilisation of electronic Repeat Dispensing
5. Answer tasks and queries regarding medication from the PCN and from community pharmacy

• identifying areas of medicines optimisation that require improvement
• supporting the implementation of change in order to promote evidence based cost-effective prescribing
• improving the quality of patient care
• improving the safety and governance of medicines in primary care
• undertaking patient medication reviews that will help patients get the best out of their medicines by identifying unmet pharmaceutical need
• managing long term conditions
• assisting in improving access and service efficiency whilst aiming to reduce waste medicines
• responsibility for encouraging the development of better medicines optimisation understanding throughout the PCN and other agencies
• promoting good practice in line with national and local guidance and formulary recommendations
• medicines reconciliation on transfer between care providers
• management of high risk drugs
• developing medicines management practices and processes
• training and education of PCN staff on therapeutics and medicines optimisation
• management of audits and implementing changes in collaboration with the wider PCN team
• implementing the directives of the PCN DES
In addition to a strong clinical knowledge, you must be have good communication, organisational and time management skills, be able to prioritise workload, build key relationships and be confident interacting with a wide range of stakeholders such as patients, GPs, practice nurses, secondary care clinicians, and CCG colleagues.

- Specialist knowledge gained through post graduate diploma in clinical pharmacy/therapeutics or equivalent qualification or level of knowledge
- Registered member of General Pharmaceutical Council

- Membership pf the Primary Care Pharmacy Association
- Membership of the Royal Pharmaceutical Society
