We have an exciting
opportunity for an experienced and highly motivated Advanced Clinical Practitioner to join our
friendly, supportive practice in Plymouth.
We are ideally seeking a qualified Nurse for this Advanced
Clinical Practitioner role, to provide care for the presenting patient from initial history taking, clinical assessment, diagnosis, treatment
and evaluation of care.
To work collaboratively with the general practice team to meet the needs of patients supporting the delivery of policy and procedures.
We are
offering 6-8 sessions per week, exact hours to be discussed/decided. The
position is at band 8a with an hourly rate between £25.99-£31.39 dependent on
experience.
We offer 6 weeks annual leave plus bank holidays.
Main duties of the job
Main responsibilities
Assess,diagnose,plan,implementandevaluatetreatment/interventionsandcareforpatientspresentingwithanundifferentiateddiagnosis.
Clinicallyexamineandassesspatientneedsandplanclinicalcareaccordingly.

Prescribeandreviewmedicationfortherapeuticeffectiveness.

Provideinformationandadviceonprescribed,orover-the-countermedicationonmedicationregimes,side-effects,andinteractions.
Prioritisehealthproblemsandinterveneappropriatelytoassistthepatientincomplex,urgentor emergencysituations,includinginitiationofeffectiveemergencycare.
