Alcester Health Centre has an exciting opportunity for a Practice Nurse to join a friendly, hardworking and forward thinking team in Alcester, we are happy to offer a fixed term contract or a permanent position. Alcester is a busy practice of circa 5600
patients and is committed to providing modern care with traditional values.
We are looking for a team player with enthusiasm and a desire to make a difference to General Practice. To complement our team, we are now looking for an additional Practice Nurse to join us for 2 or 3 days a week.
We are paper light, IT savvy practice with a well-established administration team to provide our GPs support in general practice.
Main duties of the job
To
provide assessment, screening and treatment services and health education
advice
Provide
nursing treatments to patients in association with general practitioners or
independently using agreed protocols.
Provide general and specific health screening to
practice patients using agreed protocols.
Respond to telephone requests for consultations
and visits relating to chronic disease management.
Help in the development and
maintain protocols for the Quality and outcomes Framework chronic disease
indicators
For clinical continuity ensure
nursing cover is available for annual leave and sickness
Attend practice nurse meetings /
chronic disease management meetings
Monitoring and development of individual
management plans, for patients with Chronic Diseases, agreeing these as
appropriate with the patients and other health professionals.
Checking compliance with and adherence to
appropriate treatments using a holistic patient cantered approach.
Undertake the collection of pathological
specimens including blood samples, swabs, smears, etc.
Co-operate and participate
as required in clinical audits and research projects within the practice.

We are a research and teaching practice working with medical students from Warwick University. We have regular clinical and all staff meetings enabling the teaching to be incorporated into all levels of our team, sharing good practice and reviewing areas for development. Our aim of continuing development has enabled us to demonstrate a strong performance in QOF and CQC reports.
The on-going development approach has enabled us to tailor our processes in various aspects of everyday running of the practice, to work with the best interests of the patients and the clinicians in mind. Our experienced reception and administration team work hard to direct patients to the most appropriate care. Ensuring patients are seen by the right health care professional first time.
