Main duties of the job
Managing the on-the-day demand for acute issues.
We actively support in-house development and career progression. We have
regular clinical meetings to discuss cases, role development and provide
clinical teaching. We have in-house experience in mentoring through Enhanced
Clinical Skills, Prescribing and ACP Masters qualifications
Main duties of the job
- Assess patients and as appropriate provide definitive treatment (including prescribing medications following policy, patient group directives, NICE (national) and local clinical guidelines and local care pathways) or make necessary referrals to other members of the primary care team.
Assess patients presenting with acute illness or for review of chronic disease conditions.
Undertake home visits for acute illness or chronic disease management.
Assess, diagnosis, plan, implement and evaluate interventions/treatments for patients with complex needs and follow up as appropriate.
Diagnose and manage both acute and chronic conditions, integrating both drug- and non-drug-based treatment methods into a management plan.
Advise patients on general healthcare and promote self-management where appropriate, including signposting patients to other community or voluntary services.
Undertake the collection of pathological specimens including intravenous blood samples, swabs etc.
Support the delivery of anticipatory care plans as this process develops (e.g. monitoring blood pressure and diabetes risk of elderly patients living in sheltered housing).
Provide an alternative model to urgent and same day GP home visit.
Maintain accurate and contemporaneous health records appropriate to the consultation, ensuring accurate completion of all necessary documentation associated with patient health care and registration with the practice.
About us
We have 3 Proactive Partners here at Alvaston and a strong clinical team which includes salaried GP's, Physician Associates, ANP's, Practice Nurses and HCA's and as a dispensing practice we have a team of Clinical Pharmacists on site.We have an external team supporting us also, consisting of a Physiotherapists, Midwife, Social Prescribers, Care- coordinators and Mental health nurses. We are also a teaching practice and have registrars and students on site throughout the year.
We are actively expanding our building and team to our growing population and have a strong PPG who engage and support us in gaining patient feedback, thus helping us to identify opportunities to improve our services on offer.
We are easily accessible from the M1, A50 and A52 and a number of our team are based in Leicestershire and Nottinghamshire.
