Spelthorne Health Network PCN operates within the North West Surrey region of Surrey Heartlands Health and Care Partnership and is recruiting a clinical pharmacist to join their growing pharmacy team.
Primary Care is a rapidly changing environment and we are looking for a motivated clinical pharmacist who is adaptable and has the ability to work as part of a multi-disciplinary team. We understand the value of Clinical Pharmacy in areas such as chronic disease management, timely structured medication reviews and proactive management of patients with complex polypharmacy.
We are looking to recruit a clinical pharmacist who will be based at Sunbury Health Centre Group Practice , a forward looking training practice committed to offering high quality patient care whilst providing a friendly, supportive and enjoyable place to work.
The post holders need to be highly motivated and passionate aspiring to deliver a high quality medicines optimisation service within general practice. You will need to be able to be an efficient and effective team member building good relationships with colleagues within your allocated GP Practice, with pharmacy colleagues and other health care professionals across the system to provide a high quality patient centred seamless medicines optimisation service.
Main duties of the job
The post holder will take responsibility within the practice for areas
of chronic disease management, assist with prescription and medication queries
and undertake clinical medication reviews to proactively manage patients and
directly tackle over-medication of patients including the inappropriate use of
antibiotics. Through structured
medication reviews the post holder will support patients to take their
medicines to get the best from them, reduce waste and promote self-care.

The role is based at Sunbury Health Centre Group Practice, a 19,500 patient training practice with a varied patient population providing an interesting range of health issues located in Lower Sunbury on the banks of the River Thames.
The team at Sunbury comprises 6 Partners, 5 Salaried GP’s, 1 ANP, 2 Paramedics, 3 Clinical Pharmacists, 5 Practice Nurses, 2 HCAs, 2 Phlebotomists

The post holder will
work as part of a multi-disciplinary PCN team based at Sunbury Health Centre.
The post holder will
take responsibility within the practice for areas of chronic disease management,
assist with prescription and medication queries and undertake clinical
medication reviews to proactively manage patients and directly tackle
over-medication of patients including the inappropriate use of
antibiotics. Through structured
medication reviews the post holder will support patients to take their
medicines to get the best from them, reduce waste and promote self-care. In line with the NHS Long Term Plan
commitments the post holder will focus on particular priority groups, including
but not limited to:

Hypertension
& hypercholesterolaemia

Atrial
fibrillation

Frail
elderly
-
Care
home residents

with complex needs taking large numbers of different medications
The post holder will also
support the practice and PCN with regards to prescription and medication
queries and the achievement of PCN objectives in relation to the PCN Direct
Enhanced Service (DES). They will help support the repeat prescriptions system,
deal with acute prescription requests, medicines reconciliation on transfer of
care and systems for safer prescribing, providing expertise in clinical
medicines advice while addressing both public and social care needs of patient
in the GP practice (s).
The post holder will
provide clinical leadership on medicines optimisation and quality improvement
and manage some aspects of the quality and outcomes framework, with a
particular focus on the prescribing safety quality improvement domain, and
enhanced services.
The role is pivotal to
improving the quality of care and operational efficiencies so requires
motivation and passion to deliver excellent service within general practice.
The post holder will be
joining the current pharmacy team at Sunbury Health Centre and will work
closely with the practice’s multi-disciplinary team and other pharmacists’
within the PCN. We expect this role will evolve as the PCN develops
