Job summary
We are looking for an enthusiastic, committed and motivated Clinical Pharmacist to join the team at Leyland Primary Care Network (PCN) to develop medicines optimisation services.
You must have experience of working in a similar role in General Practice or prove you have the potential and right skill set to undertake the role.
The successful candidate will have a patient facing role in managing long-term conditions; working directly with patients to assess and treat conditions, as well as promote self-care. You should have excellent written and verbal communication skills, experience of influencing others and conducting clinical medication reviews.
The successful candidate will join our current team of Clinical Pharmacists and will work as part of a multidisciplinary team to develop and run processes for repeat prescription re-authorisation, the management of medicines on transfer of care and systems for safer prescribing.
This role will
include delivering project work for the PCN around the Investment Impact Fund
(IIF) and the medicines safety module of the Quality Outcome Framework (QOF).
The Clinical Pharmacist will be aligned to our Sandy Lane and Central Park practices but may be required to provide support to other practices within the PCN Core Practice membership.
Main duties of the job
The key aims for the clinical pharmacist are:
1. To assist in the medicines related workload with prescribing colleagues.
2. Increase safety and quality of prescribed medicines
3. Provide support on medicines related issues to the whole practice team
4. Be a point of contact for patients regarding medicines queries
They should support the following clinical work:
1. Clinical Medication Review
2. Chronic Disease Management (for example, managing hypertension/Type 2 Diabetes/Asthma/COPD patients, polypharmacy review)
3. Use appropriate CCG provided clinical templates within the EMIS Web Clinical System
4. Medicines reconciliation on transfer between care providers
They should also support the following practice development work:
Prescribing Safety, Clinical audit, Local Enhanced Services support
Lead on prescription management (systems and processes, ensuring monitoring is being done, supporting clerical staff)

To produce
evidence-based information on prescribing and medicines optimisation issues
for GP practice staff, community pharmacists and members of the public.
To act as a source
of medicines information for practice staff.
To liaise with specialist
medicines information services as required, in order to deal with more complex
queries.
To provide
medicines optimisation advice and input into the PCN and wider multidisciplinary
team.
Through structured
medication reviews, support patients to take their medications to get the
best from them, reduce waste and promote self-care.
Act as a source of
medicines information for patients (e.g. regarding doses, side effects,
adverse events, possible alternatives for out of stocks).
Prescribing
support
To use practice
clinical computer systems as required to identify and monitor prescribing
issues at a PCN level
To contribute to
implementation of prescribing action plans within the PCN
practices which promote and implement high
quality prescribing and medicines optimisation, in conjunction with ICB medicines management colleagues.
To provide support
to the PCN to achieve targets in relation to areas within the medicines optimisation
action plan and national guidelines and targets such as NSFs, NICE guidance.
To develop and
deliver training of medicines optimisation issues for practice staff.
Support adherence
with national and local guidance and pathway to improve the quality, safety
and cost effectiveness of prescribing.
Work with primary
care professional and patients to implement NICE and other evidence based
guidelines.
Clinical

To identify
patients, with a prior differential diagnosis by another healthcare professional
that would benefit from medication review optimise the medication as appropriate
utilising independent prescribing skills where appropriate.
To provide the
medication review as above in a range of settings including clinics, home visits
and care home visits.
Contribute to
reductions in medicine related hospital admissions and readmissions by
supporting patients to get the best outcomes from their medicines by
identifying and addressing medicines related issues.
Work with
community and primary care colleagues to support patients identified as high
need such as those with long term conditions.
Clinical
Governance
To be the
individual responsible for the dissemination of alerts from the Medicines and
Healthcare products Regulatory Agency, the National Patient Safety Agency and
the Chief Medical Officer. Each alert will contain an indication of the
timescales or urgency with which it should be actioned. The alert should be
distributed within this timescale. The individual will decide whether EMIS
searches need to be run in a timely manner to identify affected patients and
take appropriate actions as needed.
To contribute to
minimising the risks associated with medicines use, by reviewing prescribing
systems and identifying areas of risk, then providing training to practice staff
to modify their systems to manage these risks.
To contribute to
the development of policies and procedures to promote and monitor the safe
and secure handling of medicines throughout the practice.
Contribute to the
audit programme within the practice.
Research
and development
To participate in
the evaluation of the pilot programme by monitoring the defined indicators as
per NHS England.
To contribute to
the practice audit plan.

professional supervision and support to the other GP clinical pharmacists
within the PCN as they join over the next few years.
To encourage the
sharing of good practice and peer review within the local PCN clinical
pharmacist network.
