Job summary

We are looking to appoint 3 paramedics to work in the Leyland Primary Care Network (PCN).

This is an exciting opportunity for an experienced paramedic to work in general practice seeing patients in surgery and carrying out home visits for housebound and care home patients across the Leyland PCN footprint.

The candidate should be comfortable working independently as well as part of a wider multidisciplinary team.

You will require excellent leadership skills and will play an important role in the development of the service.

You will participate in, and influence efforts, across health and social care to shape multi-disciplinary pathways designed to support patient choice, improve quality of life, promote self-management and assure early intervention through proactive provision of care in or as close to the patients own home as possible.

The PCN paramedic practitioner will actively support integrated neighbourhood ways of working to support standardisation for efficiency purposes.

Main duties of the job

Paramedics work autonomously within the community using their enhanced clinical assessment and treatment skills, to provide first point of contact for patients presenting with undifferentiated, undiagnosed problems relating to minor illness or injury, abdominal pains, chest pains and headaches.

They will demonstrate safe, clinic decision-making and expert care, including assessment and diagnostic skills, for patients within their own homes. Along with using a medical model to assess, diagnose and prescribe for acute medical conditions, the clinician will use a holistic approach to consider methods and interventions to prevent recurrence of problems and complete referrals to appropriate members of the wider multidisciplinary team.

Interviews will take place on 21st October, 2021.
