External Digital and Transformation Lead Drive Innovation, Improve Care

We are seeking a proactive and experienced leader to join us as an External Digital and Transformation Lead. In this key role, you will drive the digital transformation of frontline services, lead innovative projects, and build strong partnerships across Primary Care Networks, ICBs, and community organisations.

You will co-lead strategic objectives alongside the Internal Lead, with a focus on improving service access, quality, and patient experience. This includes overseeing performance initiatives (IIF, GPAD, Access), leading the use of digital systems such as EMIS, Ardens, Apex, KMCR and CQRS, and supporting integrated neighbourhood teams and population health work.

Main duties of the job

As the External Digital and Transformation Lead, your core responsibility will be leading the day-to-day delivery of services from our central hub. You'll ensure smooth operations, manage rotas, oversee staffing logistics, and maintain high standards of care, access, and patient experience. You will also provide direct line management to members of the multidisciplinary team, offering support and performance oversight.

In addition to service delivery, you will work in close partnership with your peer Digital and Transformation Lead to co-deliver wider strategic and improvement initiatives. This includes stakeholder engagement, representing the organisation and PCN at local forums, and contributing to service design, transformation projects, and digital innovation.

Together, you will lead the optimisation of key systems such as EMIS, Ardens, Apex, and KMCR, and help embed new technologies to support population health and operational improvement. We are a proactive and patient-focused Primary Care Network (PCN) based in central Rainham, made up of 8 GP practices and serving a population of over 38,000. Our PCN delivers a wide range of integrated services designed to meet the diverse needs of our community, including clinical pharmacy, diabetes and respiratory care, same-day paramedic home visiting, care coordination, social prescribing, mental health support, minor illness clinics, podiatry, physiotherapy, and advanced physiotherapy-led steroid injections.

We are proud to be an accredited clinical learning environment, regularly hosting nursing and paramedic students on university placements. Our central hub operates as the core of our coordinated service delivery, allowing us to offer responsive, joined-up care that supports both prevention and long-term condition management.
