Job summary

Working across the Five Elms PCN practices to coordinate patient care, the post holder is to provide time and support to patients, acting as a single point of contact to enable them to navigate the health care system. In this way, you will provide a more joined-up and co-ordinated care journey for the patient. You will work closely with the GPs and other primary care professionals within the PCN, to identify and manage a caseload of patients, making sure that appropriate support is made available to them and their carer and ensuring that their changing needs are addressed.

Main duties of the job

The Main duties include; Proactively identify and work with a cohort of people to support their personalised care requirements.

To provide coordination and navigation with the aid of digital tools for people and their carers across health and care services

To work closely and in partnership with the Social Prescribing Link Workers

Bring together a persons identified care and support needs and what matters to

them; explore the options to address these in a single personalised care and support plan.

Help people to manage their needs, answering their queries and supporting them to navigate their appointments.

Raise awareness of shared decision-making and decision support tools and assist people to be more prepared to have a shared decision-making conversation.

Ensure that people have high-quality health information to help them make choices about their care.

Assist people to access self-management education courses, peer support or interventions that support them in their health and wellbeing.

To participate in clinical governance activity and contribute to the improvement in quality of health outcomes through audit, risk management and Quality Improvement

Provide coordination and navigation for individuals and their carers across health and care services, working closely with social prescribing link workers, health and wellbeing coaches and other primary care roles.

Support the coordination and delivery of multidisciplinary teams within PCNs.

About us

Five Elms PCN, are seeking to collectively employ a Care Coordinator. The post holder will work closely with GP Practice Managers and staff, with GPs and with allied community health. The post holder will be employed by Five Elms PCN Ltd which is the host organisation that has been selected by the PCN to provide support for these roles but can expect to be based in practices. Management support to the post holder will be provided by the PCN Manager.

  • Good standard of general education GCSEs, NVQ, BTEC and Competent in Microsoft Office.
  • Experience in the use of the EMIS web clinical system would be beneficial.

Desirable

  • Previous Primary Care Experience

  • Experience of working in an administrative environment and as part of a team. Communication and relationship skills:
  • High standard of communication and interpersonal skills requiring tact and sensitivity and recognising barriers to understanding with clients and professionals.
  • Able to work independently and manage own workload

  • Ability to reflect on and share best practice with peers

Desirable

  • Able to travel locally as required
  • Passionate about combatting disadvantage and inequality in healthcare
