Job summary

Five Elms PCN are seeking to collectively employ a Health and Wellbeing Coach. The post holder will work closely with GP Practice Managers and staff, with GPs and with allied community health. The post holder will be employed by Norheads Lane Surgery which is the host organisation that has been selected by the PCN to provide support for these roles but can expect to be based in practices (or at home). Management support to the post holder will be provided by the PCN Manager.

Main duties of the job

Health and wellbeing coaches support people to increase their ability to self-manage, motivation levels and commitment to change their lifestyle. They are experts in behaviour change and focus on improving health related outcomes by working with people to set personalised goals and change their behaviours. They work with people with physical and/or mental health conditions and those at risk of developing them.

Formed in 2019, Five Elms Primary Care Network (PCN) is a collaboration of five GP Surgeries in Bromley, Biggin Hill, and Locksbottom, Orpington.

We pride ourselves on offering our patients the best care possible by working in collaboration with our allied health care professionals across the network.

Five Elms PCN offers and supports in-house training of Nurses, Physician Associates, and GPs, and, as an educational hub for health care professionals, can offer the best and most proactive forms of learning .

By working together we are able to offer our services at times that may be more flexible for our patients across the PCN.

They are experts in behaviour change and focus on improving health related outcomes by working with people to set personalised goals and change their behaviours. Health and wellbeing coaches can be an effective intervention for people experiencing a range of long-term conditions, including respiratory, cardiovascular (including type 2 diabetes and hypertension), and stress/low mood. They can also support people with weight management, diet and increasing activity levels. The role will require the post holder to

Proactively identify and work with a cohort of people who would benefit from health and well-being coaching interventions, and to support their personalised care requirements.

To work closely and in partnership with the Social Prescribing Link Workers and Care Coordinators

Teaching and supporting patients and carers to understand and manage their own conditions and maintain an independent lifestyle through health coaching techniques to encourage patient activation.

Ensure that people have high-quality health information to help them make choices about their care.

Assist people to access self-management education courses, peer support or interventions that support them in their health and wellbeing.

To use resources effectively to manage patient treatment in line with local guidance and make recommendations for change where improvements can be made.

To participate in clinical governance activity and contribute to the improvement in quality of health outcomes through audit, risk management and Quality Improvement

Qualifications and Experience

Essential

English and Maths GCSE or equivalent

Evidence of ability to both work autonomously and to seek support and guidance when appropriate.

Evidence of achieving targets and proactive approach

Experience working in General Practice

Proficient in MS Office and EMIS

Evidence of continued professional development

Understanding of delivering health coaching or behaviour change interventions to patients from diverse cultural or socio-economic backgrounds, and an understanding of how to tailor interventions to meet their specific needs.

Desirable

Evidence of working within a multidisciplinary team

Evidence of target-based work experience

Smoking cessation training or willingness to train

Experience with alcohol or drugs brief intervention training or willingness to train

Experience in working with groups or willingness to train

Experience in health and nutrition work with patients or willingness to train

Experience of working with patients with mental health conditions, and an understanding of how to integrate mental and physical healthcare in the delivery of health coaching.
