Job summary
Undertake a range of nursing
assessments and provide appropriate care / treatment in conjunction with GPs
according to practice policy, protocols etc. Strive to improve standards of
care and be involved in identifying practice population needs. Chronic Disease
management is an essential part of the role, with a particular focus on the
management of diabetic patients and the practice approach to this cohort of
patients to improve health outcomes.

implement the care of the patients within the surgery at specified appointment
times.

services offered to patients and to continually develop and improve the guidelines
around high quality clinical practice.
Child Health Programme
Immunisation
Travel Health Advice and
Vaccinations (If Qualified)
Monitoring of Patients with
Asthma, COPD (If Respiratory trained), Diabetes, CKD, CHD, Stroke and/ or
Hypertension in clinics within agreed clinical guidelines.
Treatment of leg ulcers
including Doppler
Cervical Cytology screening
General contraceptive advice and
Oral Contraceptive checks, including administering Depo-Provera
About us
A busy practice in the
heart of Crossgates, Leeds 15, looking after around 8,000 patients with a team
of highly skilled GPs, Nursing and Admin/Reception Teams. Our branch surgery is based in Scholes and this is a dispensing practice.
