Job summary

Castle Gardens Surgery is seeking a Practice Nurse to cover maternity leave. We are a modern and forward thinking practice, with a hugely supportive ethos. Our vacancy has arisen due to the maternity leave of one of our team, which begins in January. This is a Part time role and we can be flexible and offer various hours. The Practice advocates a friendly and positive working environment, and we're looking for an equally friendly, motivated and versatile individual to join us. Ideally we would like to employ an experienced Practice Nurse who will be able to offer chronic disease clinics in respiratory care. Please get in touch if you would like to know more.

Main duties of the job

The successful candidate should be able to work well within a team consisting of a range of different clinicians, and should have the skills to be able to work autonomously to provide the required care for each individual patient. Practice nursing is a varied and flexible role, which is constantly adapting to meet changing patient needs.The Practice Nurse will be involved in several clinical areas such as infection prevention and control, health promotion, chronic disease management (especially respiratory care), vaccination and immunisations.

Job description

Job responsibilities

The following are the core responsibilities of the Practice Nurse. There may be, on occasion, a requirement to carry out other tasks. This will be dependent upon factors such as workload and staffing levels:

a. As required, to provide routine nursing care to patients in accordance with clinical based evidence, NICE and the NSF

b. As required, to diagnose and manage acute and chronic conditions

c. To provide guidance to patients in the use of prescribed and over-the-counter medications regarding side effects and dosages

d. To co-ordinate the provision of travel medicine services

e. To request pathology services and process pathology results as required

f. To oversee the provision of chronic disease clinics, delivering patient care as necessary, referring patients to secondary/specialist care as required

g. To maintain accurate clinical records in conjunction with extant legislation

h. To ensure read codes are used effectively by all members of the nursing team

i. To ensure the appropriate maintenance and use of chronic disease registers

j. To ensure compliance with the practice chaperone policy

k. To prioritise health issues and intervene appropriately

l. To support the team in dealing with clinical emergencies

m. To implement vaccination programmes for adults and children

n. To delegate clinical responsibilities appropriately (ensuring safe practice and the task is within the scope of practice of the individual)

o. To support the clinical team with all safeguarding matters, in accordance with local and national policies

p. To deliver opportunistic health promotion where appropriate
