Job summary
We are seeking an enthusiastic and highly motivated Clinical Pharmacist to join our friendly, forward-thinking GP practice. This is an exciting opportunity to be part of a multidisciplinary team dedicated to providing high-quality patient care and supporting innovation in primary care services.
Essentially, the post holder will be an independent non-medical prescribing pharmacist and will independently assess patients, diagnose conditions, and prescribe medications for any medical condition within their clinical competence. This includes prescribing unlicensed medicines, subject to accepted clinical good practice. Consideration will be given to candidates willing to become an independent prescriber, pharmacists must complete an accredited course and receive a practice certificate from the General Pharmaceutical Council (GPhC).
The successful candidate will have excellent communication skills and adaptability to the ever-changing NHS.
The job is on part time basis for 30 hours per week over 4 days and the pay is a grade 7 equivalent (depending on skills and experience)

Job Summary
Working as part of a multi-disciplinary team, the clinical pharmacist will manage their time between patient clinical reviews, prescription management, prescribing safety, clinical audit, local commissioning initiatives, medicines reconciliation and have the opportunity to be involved in practice dispensing processes at our branch site in Kempsey.
This post is supported by the practice Lead Pharmacist and PCN Pharmacists working on site.
As part of a multi-disciplinary team the post holder undertake clinical medication reviews to proactively manage patients with complex polypharmacy and chronic disease.
The post holder will provide primary support to general practice staff with regards to prescription and medication queries. They will help support the repeat prescription system, deal with acute prescription requests, and medicines reconciliation on transfer of care and systems for safer prescribing, providing expertise in clinical medicines advice while addressing both public and social care needs of patient in the GP practice(s).

The Practice has six partners, eight Salaried GPs, Advanced Nurse Practitioners, Registrars, GP Clinical Pharmacists, dedicated Management and a strong Nursing Team, all of which are supported by skilled Care Navigators and Administrators. Haresfield Surgery is also a training practice with a longstanding relationship with the University of Birmingham Medical School.
