Job Summary

To support the practices by developing good medicine management within the practices, identifying areas for improvement and implementing change.

Operating within the practices clinical teams providing pharmaceutical advice and support to clinicians, support staff and patients and contributing to quality of patient care and operational

Main duties of the job

Duties and responsibilities

  • Carry out telephone and face to face consultations with patients.
  • Run minor illness clinics, prescribing medication as clinically indicated and within remit of qualifications and own professional registration regulations.
  • To identify target patient groups for medication reviews.
  • To carry out medication reviews in the surgery, patient home or in residential/nursing home settings.
  • Provide health education advice to patients and run medicine awareness campaigns for patient groups.
  • To provide routine chronic disease reviews for patients including review of medication.
  • To communicate and implement medication changes and alerts as advised by the medicines management team of the CCG or NICE guidelines or any other appropriate professional body.
  • To develop medication protocols for managing clinical conditions.
  • To ensure that the repeat prescribing policy and procedures are efficient and are updated and communicated whenever necessary.
  • To maintain good working relationships with Community Pharmacists.
  • To support clinical audit in the practice and carry out audits and provide reports relating to own area of expertise.

About us

The position will be based over two sites South Street Medical Centre and Elmstead Medical Clinic. We are a part of Eltham Health PCN and work in a collaborative way with other practices within our PCN. We have a robust team at both sites who will work with you to ensure that patient care is at the centre of all that we do.

We have an established wellbeing programme for staff and have an open door policy.

Person Specification

Qualifications

Essential

  • Recognised pharmacist qualification. Masters degree in pharmacy (MPharm) or equivalent
  • Registered with the General Pharmaceutical Council
  • Already be engaged with or willingness to undertake the CPPE Clinical Pharmacists in General Practice Education programme

Desirable

  • Member of the Royal Pharmaceutical Society
  • Specialist knowledge acquired through post-graduate diploma level or equivalent training/experience
  • Completion of or working towards an independent prescriber qualification

Experience

Essential

  • Minimum of 2 years post-qualification experience
  • An appreciation of the nature of GPs and general practices
  • An appreciate of the nature of general practice prescribing, concepts of rational prescribing and strategies for improving prescribing
  • Excellent spoken and written communication skills
  • Ability to build rapport and gain trust with patients
  • Ability to support and mentor staff
  • Demonstrate the ability to communicate complex and sensitive information in an understandable form to a variety of audiences
  • Confident IT user
  • Ability to prioritise own workload, work under pressure and to tight deadlines
  • Work effectively independently and as a team member
  • Ability to plan, manage, monitor, advise and review general medicine optimization issues in core areas for long term conditions
  • Recognises priorities wen problem solving and identifies deviations from the normal pattern and is able to refer to seniors or GPs when appropriate
  • Audit and reporting experience

Desirable

  • In depth therapeutic and clinical knowledge and understanding of the principles of evidence-based healthcare
  • Proficient in use of excel for audits and data reporting
  • Experience in the use of GP clinical systems especially EMIS
  • Good delegation skills
