Job summary

Bournemouth East Collaborative Primary Care Network (PCN) Ltd. comprises four like-minded practices working together in East Bournemouth, Dorset. The PCN serves a population of approximately 53,000 patients with a diverse demographic.

The post holder will work closely with the Senior Leadership Team to lead, develop and deliver a strategic digital transformation plan for BEC PCN.

You will be based at Shelley Manor and Holdenhurst Medical Centre, located within Shelley Park in easy walking distance of the sea. You will work closely with our other practices (Southbourne Surgery, Beaufort Road Surgery, Littledown Surgery).

Main duties of the job

  • Lead the planning and prioritisation of digital changes
  • Work alongside practices to understand current challenges and identify solutions
  • Develop and manage a Digital transformation plan for the PCN
  • Provide hands-on support to practices in implementing digital transformation
  • Be a conduit and connector for digital transformation with the ICS (Integrated Care Service) and related networks
  • Support staff to identify and participate in relevant training and/or professional development opportunities
  • Support practices to optimise the use of digital solutions, technology and new ways of working
  • Advise on innovative opportunities to support practices to maximise benefits from innovation, through the sharing of learning and best practice
  • Develop and implement mechanisms for assessing the impact of agreed changes
  • Provide feedback to PCN leadership team and member practices on progress
  • Review and improve the PCN's digital maturity
  • Facilitate collaboration between practices and the wider system to support the delivery of care to patients
  • Use data, and improve data quality, to underpin transformation and improvement
  • Improve adoption of new technology to enhance patient access and experience, increasing PCN productivity
  • Responsible for a range of digital transformation initiatives for the PCN, managing priorities and inter-dependencies
  • Ensure that digital transformation is embedded and aligned with system and national strategies
  • Identify opportunities for digital optimisation across the PCN

Job responsibilities

The Digital Lead will work with clinical and support teams within Primary Care Networks to identify, scope, plan, and lead digital transformation, drawing insight and expertise from and staff at all levels.

The post holder willuse data and local insight to identify opportunities to drive transformation & improvement with a view to improving:

  • Patient access
  • Staff and patient satisfaction
  • Optimising the use of digital and new ways of working
  • Improving the efficiency and sustainability of general practice
The post holder will work closely with practice and PCN colleagues to understand and identify challenges, agree priorities, and support the delivery of improvement. The post holder will develop strong relationships with PCN colleagues, member practices, ICB colleagues and wider ICS networks to support the planning and delivery of digital transformation that will provide benefits at individual practice level and at the level of the PCN.

The post holder will work closely with ICS colleagues and networks to ensure local digital transformation, strategy, plans and activity are aligned to the ICS Primary Care priorities including:

  • Optimisation of digital and new ways of working
  • Retention & development of multi-professional teams
  • Hardwiring the system to support change through Quality Improvement

The post holder will work to coordinate with and bring in additional specialist expertise when required to deliver transformation plans, e.g., analytics, evaluation, policy, digital, workforce, estates etc.

The post holder will help to develop strategy related to digital transformation and improvement, aligning this with larger scale strategy and initiatives; but will focus their time on the planning, implementation and evaluation of practical improvements that make a difference to patients and staff, ensuring they address health inequalities and championing diversity and inclusion. This will be balanced with an operational hands on approach working alongside and supporting colleagues to deliver transformation.

The post holder will work autonomously, connecting with the PCN digital team (Digital Care Coordinator, Data Ambassador, Digital Champion) to deliver PCN digital projects such as digital triage, patient flow management, automation opportunities, development of new shared care systems and optimising the use of clinical systems.

The post holder will help the PCN adopt new technology to improve access to care and enable PCN staff to work more effectively to support the sustainability of general practice services.

Information and data management

The post holder will need to have skills in relation to understanding and using data to drive improvement and assess impacts. These skills will be used to:

  • Take a data driven approach, looking at evidence and information available to support understanding of challenges, identify possible solutions and track the impacts of changes that have been made.
  • Bring together data from a variety of sources and use information to develop insights into working practices, effectively tailoring content to meet the needs of the audience.
  • Effectively present and communicate data to practices and PCN staff at all levels to increase understanding of operational challenges.
  • Interpret data and highlight variation to support open discussion about where variation is and is not warranted.
  • Help PCN and practice staff understand the limitations of particular data sets and where other information may be available to supplement understanding of issues.
  • Support and encourage a focus on data in understanding the effectiveness of changes made
  • Seek and secure analytical resource to support the PCN in developing and implementing appropriate data and information sharing mechanisms.
  • Use data to track and assess impact of changes made to operational processes, and to track trends and developments over time.

Communication and engagement

With responsibility for developing and supporting delivery of transformational projects across the PCN, the post holder will require expertise in communications and engagement, and specifically in bringing people together and gaining support and buy-in to change.

Build good working relationships with internal and external stakeholders.

Initiate and participate in development of effective systems of communication and co-operation between member practice.

Ensure that changes are aligned with agreed clinical priorities and have ownership and engagement from clinical leaders and staff.

Identify best practice from other areas that could be applicable to the local PCN and communicate this to staff.

Make connections across the PCN to support learning between practices and sharing of best practice.

This expertise will be used to:

Maintain a good knowledge of emerging policies from the Department of Health and Social Care, NHS England and Improvement and other relevant government departments and agencies, and support PCN staff to understand and interpret these locally.

Work with the Integrated Care Board (ICB) to support the development of overall primary care transformation strategies and their delivery.

Support the PCN and member practices to adapt and apply specific policies and initiatives to local needs and to prioritise national or system level initiatives based on local requirements.
