Job summary

The OT will be part of the Primary Care Network team and will work closely with all the 5 GP practices within the Andover PCN. we are looking for an OT with at least 5 years qualifying experience to support new innovative projects within the primary care team. A background in frailty, managing long term conditions or mental health would be desirable but other relevant experience would also be considered

Main duties of the job

Working independently, the post holder will manage a varied and complex caseload providing OT assessment and intervention to patients aged 16 and over who are identified as needing occupational therapy. The post holder will be required to work as part of the multi-disciplinary team offering advice & recommendations to the team based on the outcome of assessments & interventions carried out with patients. The post holder will be expected to work within the limits of their professional competence, skills and knowledge base.

About us

Primary Care Networks form a key building block of the NHS long-term plans. Bringing general practices together allow us to provide a wider range of services to patients, whilst also decreasing the workload on GPs. This improves the ability of practices to recruit and retain staff, manage finances and facilities more efficiently, and to more easily integrate with the wider health and care system.

Andover Primary Care Network is a collaborative project involving the following GP Surgeries: Adelaide Medical Centre, Andover Health Centre Medical Practice, Charlton Hill Surgery, Shepherds Spring Medical Centreand St Marys Surgery.

As we come out of the Covid Pandemic and demand pressures on primary care continue to grow, the time for reorganisation of the NHS and a political appetite for transformation of services is increasing. Primary Care in Andover suffers these pressures on top of longer standing issues of socio-economic deprivation and the resulting health problems, which have been exacerbated by the pandemic.

The PCN offers additional roles working across practices, providing alternatives for patients to seeing a GP. Using pharmacists and care-coordinators to proactively manage our elderly and complex patients. We expect the PCN to continue to develop and improve patient care in Andover, and to create opportunities for innovative ways of promoting wellbeing.

Primary Responsibilities

assess, plan, implement, and evaluate treatment plans, with an aim to increase patients occupational functioning

work with patients through a shared-decision making approach to plan realistic, outcomes-focused goals;

undertake both verbal and non-verbal communication methods to address the needs of patients that have communication difficulties;

work in partnership with multi-disciplinary team colleagues, physiotherapists and social workers, alongside the patients' families, teachers, carers, and employers in treatment planning to aid rehabilitation;

where appropriate, support the development of discharge and contingency plans with relevant professionals to arrange on-going care in residential, care home, hospital, and community settings;

periodically review, evaluate and change rehabilitation programmes to rebuild lost skills and restore confidence;

as required, advise on home, school, and workplace environmental alterations, such as adjustments for wheelchair access, technological needs, and ergonomic support;

advise patients, and their families or carers, on specialist equipment and organisations that can help with daily activities;

help patients to adapt to and manage their physical and mental health long-term conditions, through the teaching of coping strategies; and

develop, implement and evaluate a seamless occupational therapy support service across the PCN, working with community and secondary care where appropriate, and aimed at continuously improving standards of patient care and wider multi-disciplinary team working.

work with other colleagues particularly the social prescribing service to connect patients to community-based activities which support them to take increased control of their health and wellbeing;

provide education and specialist expertise to PCN staff, raising awareness of good practice occupational therapy techniques

ensure delivery of best practice in clinical practice, caseload management, education, research, and audit, to achieve corporate PCN and local population objectives
