Job summary
The
average GP appointment of 10 minutes does not provide the required time for
quality mental health care. Routine
appointments for common mental health problems can often take longer. An
alternative practitioner, with appropriate skills and training, could
potentially manage many of these patients the Mental Health Practitioner (MHP) will
be recognised as an expert practitioner & will advise the Primary Care Team
of safe and effective modalities of care & treatment.
Mental Health Practitioners will improve the quality of patient
care by providing comprehensive patient centred mental health assessments & triage appropriate referrals to both secondary care mental health & psychological therapies.
As part of the wider Primary Care Team, they can assess, diagnose,
treat and discharge without the patient needing to be seen by a GP. This
process will release GPs to work with patients who have complex physical
health related issues.
They will work towards improving the quality of prescribing
interventions for common mental health problems such as depression & anxiety
by using evidence-based practice, reduce the number of inappropriate referrals
and hospital admissions, & support patients to self-care, whenever possible.
The role will mainly work with people who experience common mental
health problems, such as mood & anxiety disorders & all interventions are
in line with NICE guidelines and evidence based.
Main duties of the job
It is recognised that people who experience common mental health
problems have many social & health related problems & can present
with a significant risk profile. The MHP will be
instrumental in managing risk & also working innovatively alongside partner
agencies, for example in respect of the development of Social Prescribing,
Neighbourhood Engagement, Community Asset Mapping, the identification of carers & our practice GPs & healthcare team.You will be improving
links across the health & social care economy & pivotal in the delivery of
psycho education. You will need exceptional
organisational skills as you will be dealing with fluctuating levels of demand
across the Practice in respect of support tasks that require to be undertaken. You will also monitor people with severe mental illness who are deemed stable & need monitoring under the shared care protocols, as they may be prescribed antipsychotics or mood stabilisers. A Primary
Care MHP will have the skills and knowledge to take a
history, examine & assess patients & decide on high-quality care, which
involves early intervention or a planned system of collaborative care.You will be required to link in with
other health and social care providers across the Isle of Wight to develop
strong links; & will also be required to provide data evidencing the work
that you undertake. You will be required to formulate reports including
metrics which in turn will be used to evidence the overall outcomes of the work
undertaken.
