PRACTICE NURSE

Job responsibilities

The post holder will provide general practice nursing duties including; chronic disease management, immunisations (adhering to current national guidelines), performing cervical smears (adhering to current cervical cytology register requirements) and run general nurse led clinics.

Main Duties and Responsibilities:

Ensure that patients receive high quality clinical care, delivered in a timely manner.

Ensure complete and accurate documentation of each and every patient contact.

Work as an independent practitioner and as part of multidisciplinary and multi-agency teams in order to ensure patients needs are met.

Provide general practice nursing duties including immunisations (adhering to current national guidelines), performing cervical smears (adhering to current cervical cytology register requirements), run nurse led clinics and adhere to and implement infection control guidelines.

Provide clinical advice to Reception staff where required.

Offer support to junior staff and learners.

Ensure that as a nursing team we are prioritising people, practising effectively and safely and promoting professionalism as per nursing code of conduct.

Teaching and support

Contribute to the development and maintenance of an excellent learning environment.

Work with a GP Lead/mentor to support clinical practice and development of knowledge/skills.

Adhere to professional NMC registration requirements.

Participate in defining own developmental plan to maintain and develop own clinical skills and ensure own professional development.

Undertake and complete all statutory and mandatory training.

Participate in an appraisal and regular performance reviews.

Support the development of the HCAs.

Nursing protocols and guidelines - ensure that the Infection control and prevention protocol is up to date and available electronically for all practice staff to access.

Professional

Adhere to the NMC Code of Conduct and at all times work within the scope of professional practice.

Ensure that professional practice adheres to organisational policies, procedures and guidelines.

Maintain a professional manner and act as a role model and mentor for junior staff including trainees.

Maintain confidentiality with regard to information pertaining to patients and staff.

Communication

Communicate effectively with patients and families regarding complex information about their care

Provide advice and information to patients, carers and their families where appropriate

Communicate effectively and work collaboratively with medical, nursing and allied health professionals to ensure delivery of a co-ordinated service.

Ensure accurate documentation and record keeping in line with local policies and NMC guidelines.

Promote and demonstrate effective communication networks within the organisation, with partner organisations and other service providers.

Clinical Governance

Document significant events.

Adhere to Chipping Norton Health Centre procedures and guidelines.

Respect patient confidentiality and comply with Information Governance training.

Demonstrate commitment to flexible working patterns, to meet the needs of the service and staff, providing cover for colleagues.

The post-holder must have their own car and a full driving licence and have appropriate insurance for using their vehicle for work-related purposes.

Experience

Essential

• We are looking for a motivated experienced and enthusiastic nurse to join our friendly, practice nursing team. You need to be a team player who will provide high quality care to all our patients.
• You will need to work independently but also within the multi-disciplinary team and maintain excellent communication skills.
