Job summary
Oakfield Street Surgery are looking to recruit a
Medical Records Summariser to join our busy team. The position available is
for 15 to 22.5 hours per week.
You
must be organised, self-motivated and forward thinking with excellent administrative
skills. The candidate must have worked in a clinical/clinical
support role in primary or secondary care or have previous summarising
experience with specific training. You
must be computer literate as you are required to input accurate information
into the electronic patient record, ideally with experience in using VISION/EMIS
or a similar clinical system. Knowledge of medical terminology is required.

The
main responsibilities of the post-holder will be to summarise and read code all
patient records received at the surgery with relevant information from the
patients clinical records into the practice clinical system (Vision). Working
strictly in accordance with practice summarising and clinical coding protocols.
Main duties of the job

Work in accordance with practice summarising and clinical coding
protocols.
Log all new incoming patient records.
Review medical records and produce an accurate summary of the
patients medical history, within the practice protocols and within agreed
timescales.
Enter Read Code information onto the patient notes on the practice
computer system, adhering to QAIF requirements.
Record sensitive information e.g., child protection and
safeguarding.
Enter clinical data from letters & other information received
from care providers, according to practice protocol.
Maintain structure in the computerised record when recording and
read-coding new information.
Send patient notes to storage on completion.
Code letters & other information received from care providers
using the agreed practice protocols.
Managing electronic GP2GP links.
Adhere to practice policies and procedures including quality and
diversity, H&S, confidentiality and GDPR.
You may be asked to undertake other functions as necessary to meet
the needs of the business. Training will be provided as required.

Oakfield Street Surgery is a busy practice in the heart of Ystrad Mynach town centre. We have a large team, including 6 GP Partners, 4 Salaried GP's, 1 ANP, a nursing team of 6 and numerous reception and administrative staff. We are proud to be a training practice of both GP registrars and medical students. We aim to provide a professional and efficient service to our 13,500 patients, GP's, Health Service professionals, outside agencies, colleagues and other staff
members.
