Job summary

We are looking for a dynamic Acute Primary Care Practitioner to join our multi-disciplinary team and broaden our clinical skill-mix. This will be an extremely varied role and the successful candidate will be a qualified nurse practitioner or specialist paramedic who will treat patients for Acute Illness over the telephone, at the surgery and in the patients own home as part of our Same Day Duty Service (SDS).

We are seeking to appoint someone with a caring nature and proactive professional manner to provide a high standard of care for our 12,183 patients. This is a fantastic opportunity for a professional with good communication skills, and a commitment to working as part of a strong and effective nursing, same day duty team and wider practice team.

Main duties of the job

Ideally, the successful applicant will be expected to undertake same day care within the practice, working as an autonomous practitioner, providing clinical care (including telephone triage, diagnosis, treatment, referral, review) as appropriate, utilising a range of advanced skills and knowledge for patients presenting with undifferentiated and undiagnosed conditions. There will be flexibility for the right candidate and secondments will be considered. This is a new and exciting role within our practice and we are look for the right person who can help evolve and embed it within our practice.

Job responsibilities

To be largely responsible for the provision of emergency duty support, working as part of the practices multidisciplinary team, delivering care within their scope of practice to the entitled patient population. The Acute Primary Care Practitioner will be required to work autonomously, as part of the duty team, in the practice and in the community (home visits), providing a range of services such as history taking, clinical assessment, diagnosis, treatment, telephone triage and referring patients appropriately, as well as supporting the management team in the reviewing of clinical policy and procedure

The following are the core responsibilities of the acute primary care practitioner. There may be on occasion a requirement to carry out other tasks; this will be dependent on factors such as workload and staffing levels. The practitioner will work within their scope of clinical practice and:

1. Examine, assess and diagnose patients and provide clinical care/management for patients presenting with an undifferentiated diagnosis.

2. Triage patients and provide the necessary treatment during home visits and telephone

3. Triage and treat or refer patients who attend clinic.

4. Identify, signpost or refer patients at risk of developing long-term conditions, preventing adverse effects on the patients health

5. Provide routine care to patients as required in accordance with clinically based evidence, NICE and the NSF

6. Collect pathology specimens as required

7. Maintain accurate clinical records in line with extant legislation

8. Ensure that read codes are used effectively

9. Ensure continuity of care, arranging follow-up consultations or reviews as necessary

10. Review the effectiveness of the treatment provided, making changes where necessary to improve patient outcomes

11. Chaperone patients where necessary

12. Prioritise health issues and intervene appropriately

13. Act as one of the lead clinicians when dealing with clinical emergencies

14. Recognise, assess and refer patients presenting with mental health needs

15. Support patients in the use of their prescribed medicines or over-the-counter medicines, regimens, side-effects and interactions. (within own scope of practice)

16. Prescribe and review medication for therapeutic effectiveness, appropriate to patient needs and in accordance with evidence-based practice and national/practice protocols

17. Prioritise health problems and intervene appropriately to assist the patient in complex, urgent or emergency situations, including initiation of effective emergency care

18. Liaise with external services/agencies to ensure that the patient is supported appropriately (vulnerable patients, etc.)

19. Ensure that they adhere to the relevant patient group directives and local clinical pathways at all times

20. Support the clinical team with all safeguarding matters, in accordance with local and national policies

21. Understand practice and local policies for substance abuse and addictive behaviour, referring patients appropriately

22. Deliver opportunistic health promotion where appropriate

Secondary responsibilities

In addition to the primary responsibilities, the practitioner may be requested to:

a. Support the practice audit programme, undertaking audits when necessary

b. Support junior members of the team, providing guidance when necessary

c. Participate in local initiatives to enhance service delivery and patient care

d. Support and participate in shared learning within the practice

e. Continually review clinical practices, responding to national policies and initiatives where appropriate

f. Participate in the review of significant and near-miss events, applying a structured approach, i.e. root cause analysis (RCA)

g. Take personal responsibility for own learning and development, including the requirement to maintain currency, achieving all targets set in own Personal Development Plan (PDP)
