Job summary

We are looking for an Advanced Paramedic Practitioner to join our growing practice in the centre of the beautiful market town of Farnham.

We are a forward thinking, high achieving practice, offering excellent professional development opportunities.

We enjoy a great partnership with our PCN and established Integrated Care Team, which support our clinical activity. You will also benefit from working occasional sessions at our Same Day Urgent Access Centre at Farnham Hospital.

The Advanced Paramedic Practitioner role, will be a registered Paramedic with post graduate qualifications, including the Independent and Supplementary Prescriber qualification and HCPC registration, acting within their professional boundaries.

Main duties of the job

You will work as an autonomous practitioner, responsible for the provision of services to the practice population, delivering an excellent standard of clinical care. The post holder will use skills, knowledge and competencies as an Advanced Paramedic Practitioner in order to be responsible and accountable for managing patient caseloads, including treatment, referral and admission of patients as appropriate. Furthermore, you will contribute to the effective management of the practice, leading by example, maintaining a positive, collaborative working relationship with the multidisciplinary team.

Our current list size is 13,500 and our practice comprises a team of 10 doctors, (including 3 partners), 4 nurses, 3 HCAs and a wide variety of additional clinical roles (Physicians Associate, Clinical Pharmacists, Dietician, Social Prescribers).

  • an established Integrated Care Team including frailty, mental health and proactive care services

Job description

Job responsibilities

  • Alongside the multidisciplinary team, to provide a first point of contact for patients presenting with undifferentiated, undiagnosed problems. Making use of skills in history taking, physical examination, problem-solving and clinical decision-making to establish a diagnosis and management plan.
  • To provide assessment, treatment and diagnosis at point of first contact by attending to patients.
  • To make professionally autonomous decisions for which he/she is accountable
  • To refer patients to an alternative care setting or treat and discharge as appropriate
  • To instigate necessary invasive and non-invasive diagnostic tests or investigations and interpret findings/reports
  • Perform specialist health checks and reviews within their scope of
  • practice and in line with local and national guidance
  • Perform and interpret ECGs
  • Support the delivery of anticipatory care plans and lead certain services (e.g., monitoring blood pressure and diabetes risk of elderly patients)
  • Generate EPS and FP10 prescriptions within scope of practice, considering local and national guidelines.
  • To ensure that professional standards are maintained and within the guidance provided by the Department of Health, the Health and Care Professions Council (HCPC) and the College of Paramedics (COP)
  • To support network implementation of agreed service changes and pathways, working closely with other staff members, support and deliver local improvement programmes aligned to national and local priorities.
  • To contribute to the service achieving its quality targets to sustain high standards of patient care and service delivery
  • To participate in identification of community health needs and develop patient/family centred strategies to address them
  • To help develop and set up new patient services and participate in initiatives to improve existing patient services

  • Completed a two-year consolidation of learning period when a newly qualified paramedic
  • Completed further three years experience as a band 6 (or equivalent) paramedic and attained Level 7 capability in paramedic areas of practice
  • Completed and been signed off formally within the clinical pillar competencies of the Advanced Clinical Practice Framework.
  • Assess and triage patients, including same day triage, and as appropriate provide definitive treatment (including prescribing medications following policy, patient group directives, NICE (national) and local clinical guidelines and local care pathways) or make necessary referrals to other members of the primary care team.
  • Able to manage undifferentiated undiagnosed condition and identify red flags and underlying serious pathology and take appropriate action
  • Use complex decision making to inform the diagnosis,
  • Investigation and complete management of episodes of care within a broad scope of practice
  • Actively take a personalised care approach and population centred care approach to enable shared decision making with the presenting person
  • Have completed the relevant training in order to provide multi-professional clinical practice and CPD supervision to other roles within primary care, for example first contact practitioners and the personalised care roles
