Job summary

The Wall House Surgery is a dynamic, digitally enabled team committed to population health, preventative care, and patient empowerment. We are currently seeking a dedicated and compassionate Nurse Associate to join our innovative care team. This role involves working within the practice as well as visiting housebound patients to conduct chronic disease reviews in their homes.

Main duties of the job

  • Conduct Home Visits Provide face-to-face care for housebound patients in their own homes or in residential care settings.

  • Long-Term Condition Reviews Carry out annual health checks for patients with chronic conditions, including:

    • Blood pressure and pulse monitoring

    • Foot checks

    • Blood sample collection

    • ECGs (following training)

    • Spirometry or FeNO testing (following training)

    • Recording height, weight, and BMI

  • Vaccination Delivery Administer vaccinations as required, following appropriate training and competency assessment.

  • Health Promotion Deliver lifestyle advice, support self-care education, and signpost to relevant health and community services, adopting a holistic approach to patient care.

  • Proactive Care Planning Collaborate with care coordinators, GPs, pharmacists, frailty leads, community matrons, and anticipatory care teams to support personalised care planning for high-risk or vulnerable patients.

  • Digital Care Management Use electronic tools to help identify and prioritise patients most in need of care. Maintain accurate clinical records using a provided laptop and clinical systems.

    • Registered with the NMC

    Experience

    Essential

    • Confident and compassionate when working independently in patients homes
    • Experience or willingness to work with older people and individuals living with frailty
    • Strong communication skills with the ability to build rapport with patients and their families
    • Competent in performing basic clinical observations and delivering nursing care tasks
    • Full UK driving licence and access to a car for community visits

    Desirable

    • Experience in primary care, community nursing, or long-term condition management
    • Training or experience in ECGs, spirometry, FeNO testing, or vaccine administration
    • Understanding of population health, anticipatory care, and personalised care planning
    • Familiarity with EMIS or similar clinical systems
