Job summary

Aughton Surgery is looking to recruit an experienced prescribing Clinical Pharmacist for 20 hours per week.

We are happy for some hours to be done remotely, but we do expect the post holder to work in person at the surgery as well. We can accommodate all working hours to be done at the surgery on a weekly basis if this is the applicants preference.

The applicant will be a prescribing clinical pharmacist who acts within their professional boundaries, working alongside a team of pharmacists in general practice.

They will improve patients health outcomes and the efficiency of the primary care team by providing direct, accessible and timely medicines and expertise.

The post holder will be an integral part of the general practice team, as well as part of a wider pharmacist network and multi-disciplinary team. They will work to optimise medication issues to improve patient care and safety, and support clinical staff in the management of patients.

As part of their employment, the post holder must hold an independent prescribing qualification.

Main duties of the job

  • Repeat prescription management including dealing with queries raised by our prescriptions co-ordinator.
  • Ensuring patients medication reviews are up to date.
  • Medicines reconciliation from clinical correspondence.
  • Structured medication reviews.
  • Responding to and actioning tasks from clinical and administrative staff.
  • Being responsible for actioning MHRA drug alerts.
  • Reviewing and re-authorising newly registered patients medication from GP2GP transfers.
  • Blood pressure management
  • Medication management of patients with long term conditions
  • Dose amendments to patients medication based on blood results (e.g. Levothyroxine)

This list is not exhaustive and other duties may be required at the need of the surgery.

About us

Aughton Surgery has 5 partners (3 GP's, 1 Practice Manager and 1 Finance), as well as 3 Salaried GP's, 1 Physician Associate, 1 Practice Nurse and 1 HCA. We are also a training practice, so we regularly have GP registrars, medical students and student PA's as part of our team as well.

Our clinical staff are assisted by a friendly and knowledgeable admin/reception team.

All staff are highly supportive, and the surgery is a very enjoyable environment to work in

Essential

  • GPhC registered Pharmacist
  • Qualified from an approved 18 month training pathway (For example CPPE)
  • GPhC Independent Prescribing Qualification

Desirable

  • Minor ailments certification
  • Member of the Royal Pharmaceutical Society
  • Medicines management qualification

Experience

Essential

  • Minimum of 2 years working as a clinical pharmacist within General Practice
  • Appreciation of the nature of primary care prescribing
  • Awareness and knowledge of common acute and chronic conditions that are likely to be seen in primary care

Desirable

  • Clinical and therapeutic knowledge, and an understanding of the principles of evidence-based healthcare
  • Broad knowledge of General Practice and the way that primary care works, and how it is funded
