Job summary

Greyland Medical Centre is a small single GP practice based in the North of Manchester. We are a small friendly team. We are looking for an experienced Practice Nurse. Duties will include womens health (cytology), Chronic disease management, blood tests etc.

The post is part time 12 hours per week ideally over two days and ideally Monday and Wednesday. Salary will be in accordance with experience.

The Practice works on the Vision system.

Job description

Job responsibilities

Main Purpose of the Post:

As a member of the practice team to provide nursing services to patients of the practice

Job Summary:

To assist the medical personnel in the care of the practices patients, to include chronic disease management, participation in local and national enhanced services as well as preventative care, health screening and patient education.

Chronic disease management to include the following areas, either as dedicated clinics or within normal clinic sessions:

Diabetes

Chronic Heart Disease

COPD

Spirometry

Asthma

Hypertension

Taking an active role in the preparation and review of care plans for those patients who are at risk of hospital admission, as well as participating in any other enhanced services as required.

Other duties to include:

Cytology

Venepuncture

Assisting with minor operations, female health clinics when required

Routine/Childhood immunisations

Administering flu vaccines in season

Travel health including travel vaccinations

NHS Health checks

New patient examinations

ECGs

Chaperoning and assisting patients who are being examined by another clinician when appropriate
