Job summary
Due to an expanding patient list and our desire to remain
proactive in our approach to patient care, the Strand Medical in Goring-by-Sea
have an exciting opportunity for someone that is motivated, organised and
proactive to manage our housebound patients care.
Main duties of the job
The role will include completing chronic illness annual reviews such as diabetes, asthma, COPD, hypertension and dementia, giving vaccines such as shingles, pneumovax and flus to those that are eligible, as well as odd jobs such as repeating a BP following meds change.
About us
Strand Medical is a training practice with four
Partners, one salaried GP, five Advance Nurse Practitioners, two Paramedic
Practitioners and a nursing team led by two Lead Nurses in Goring-by-Sea,
serving the south coast from West Worthing to Ferring with a Practice
population around 15,600.
The Reception and Administrative teams are led by a
Practice Business Manager and Deputy Practice Manager. The Reception team is
led by Head of Reception, supported by three Team Leaders whom oversee a
different aspects of the teams work.
We have a modern team leader approach, which
maintains excellent levels of communication and efficiency. Although we are a
large, busy Practice, we place great value on a friendly working atmosphere and
would expect any new post holder to complement this. We work as a multi-skilled
team, recognising and valuing everyones contribution to the effective care of
our patient population.

We have just under 300 housebound patients, so this in an
excellent opportunity for the right individual to help plan and manage this
cohort of patient care across the year. We would expect approximately 30
reviews to be completed per month, leaving 2 months to allow for flu
distribution. You would be line managed by our Lead Nurse and support would
also be given by a team of well-established and experienced in-house clinicians
as well as from a wider MDT to assist with the more complex patients.
Ideally you will be proficient in chronic illness reviews,
or have experience of general practice, however if not, we are willing to offer
training to the right individual.
