Job summary

Parkgate Surgery are looking to recruit a Practice Nurse to join our busy GP practice. 

We are offering a part-time role of approximately 24 hours per week, to be worked over 4 days. 

Main duties of the job

Candidates should be experienced in all areas of Chronic Disease Management, Cervical Cytology and Vaccination and Immunisation. A specialist interest in Diabetes and COPD would be desirable but not essential.

You will work with the full support of the practice team together with benefitting from the substantial networking opportunities across our other practices in the area. This is a great opportunity to join a well established team.

There are opportunities to develop special interests for those with drive and enthusiasm.

The ideal applicant will have experience in the Quality and Outcomes framework and other targets required of modern day general practice, confident with IT and management of processes in a rapidly changing environment.

You will have excellent communication skills, a compassionate and friendly nature who can operate in a busy environment with a range of people and professions, as you will be working with our clinical and administration teams.

Job description

Job responsibilities

  • Develop, implement and embed health promotion and well-being programmes

  • Identify, manage and support patients at risk of developing long-term conditions, preventing adverse effects on patients’ health

  • Implement and evaluate individual treatment plans for chronic disease patients

  • Request pathology services as necessary

  • Process pathology results as required

  • Provide chronic disease clinics, delivering patient care as necessary, referring patients to secondary/specialist care as required

  • Maintain accurate clinical records in conjunction with extant legislation

  • Ensure read codes are used effectively

  • Maintain chronic disease register

  • Chaperone patients where necessary

  • Assist GPs with minor surgery when required

  • Prioritise health issues and intervene appropriately

  • Support the team in dealing with clinical emergencies

  • Recognise, assess and refer patients presenting with mental health needs

  • Implement vaccination programmes for adults and children

  • Support patients in the use of their prescribed medicines or over-the-counter medicines (within own scope of practice)
