Job summary

Due to increasing demand on our services, Ashworth Street Surgery is looking for a part time Practice Nurse to work 24 hours over 3 days to complement our existing clinical team. Experience is highly desirable as we are looking for an immediate start. The ideal candidate will have experience of QOF, IIF, local and national targets.

Main duties of the job

You must be proficient in:

  • Chronic Disease Management / reviews i.e Diabetes, respiratory, Dementia, hypertension, RA, Cancer reviews etc
  • Women's health including Cytology 
  • Child health including vaccinations 
  • Basic wound management
  • General Immunisations 
  • Travel Health (including immunisations covered within the NHS contract)
  • Visit patients in their own home as required where appropriate to do so
  • Order and maintain stocks 
  • Emergency Drug checks 

The applicant must possess current NMC registration and a full U.K driving licence.

About us

We are a large GMS training Practice based in Rochdale serving over 15000 patients. We are looking to employ someone to enhance our existing Nursing team. We are a well-established experienced team, we have 8 GP Partners, 2 salaried GP's a varied number of trainees, 1 ANP, 1 Community Nurse Practitioner, 2 Practice Nurses, 2 Health Care Assistants, 1 Phlebotomist and an experienced administration and reception team, supported by the Office Manager/Practice Operations Manager.

Job description

Job responsibilities

Management and monitoring of chronic diseases and LTC including:

CHD, Hypertension, Diabetes, Asthma, COPD, RA, Dementia, Sexual Health, Cancer Review

· To provide nursing services to patients of the practice including the development and promotion of health screening programmes, within the professional competence of the post holder. The post holder will work in accordance with the NMC code

· Provide nursing treatments to patients in collaboration with G.P.’s or independently to agreed protocols

· Child Health surveillance clinics

· Participation in National Screening and Immunisation Programmes, cervical screening, childhood immunisations, immunisation within at risk populations

· Travel advice (including immunisations covered within NHS contract)

· Visit patients where appropriate in their own home

· Maintain accurate patient records, statistics and equipment used

· To order and maintain stocks of vaccines and injectables personally administered by GP's and Nursing team

· To maintain professional development and education

· To liaise with outside agencies to provide a good standard of service to the Practice population

· Responsible for promoting and safeguarding the welfare of the patients you are responsible for or come into contact with

· The ability to self motivate, organise and prioritise workload

· Good communication skills

· The post holder must be in possession of a current NMC registration

· The post holder must hold a full U.K. driving licence
