Job summary

An exciting opportunity to flex your skills in a role that will help transform patient care in a primary care setting. Our ideal candidate will have great interpersonal skills to engage and influence patients using expert knowledge of medicines for specific disease areas. You will also support and work alongside the practice team and across the clinical network of 11 rural practices. The role is pivotal to improving the quality of care and improve pathways so requires motivation and passion to deliver excellent service. We are looking to recruit a full-time Band 7/8a candidate who we can support to become a prescriber but will consider applicants looking for some flexible working.

Main duties of the job

Daily tasks will include undertaking of clinical structured medication reviews to proactively manage patients including those with complex polypharmacy, potentially developing a specialist area of chronic disease or pain medicines management. You will provide primary support to the practice staff for prescription and medication queries and support the development of practice repeat prescription systems and lead medicines reconciliation on transfer of care ensuring safe systems for medicines. You will provide expertise in clinical medicines advice while addressing both public and social care medicines needs of patients in the GP practices. You will provide clinical leadership on medicines optimisation and quality improvement.

Forest Health Care is a friendly, democratic partnership of 4 supported by 2 salaried GPs, an ANP, prescribing nurses and a large team of practice nurses and HCAs. Nestling in the glorious Forest of Dean, our Cinderford surgery serves the surrounding town and rural communities. Situated in spacious, environmentally friendly new premises, we occupy the first purpose-built, dementia-friendly surgery in the UK. Patients can also access a full range of services at our branch site in local village of Ruardean where every room has a stunning view. We are both a training practice and a dispensing practice with a list size of approx. 7800 and additionally we are currently contracted to run the local community hospital. We are a busy practice looking for a dynamic new addition to our loyal and longstanding team.

Qualifications

Essential

  • -Masters degree in pharmacy (MPharm) (or equivalent)
  • -Mandatory registration with General Pharmaceutical Council
  • -Evidence of continuing professional development

Desirable

  • -Hold a clinical pharmacy MSc or equivalent standard qualification suitable for the post
  • -Independent Prescriber / commitment to become an Independent Prescriber

Experience

Essential

  • -Experience of working in primary or secondary care
  • -Minimum of 5 years post-registration experience (less would be considered for the right candidate)
  • -Knowledge of evidence-based practice and clinical governance
  • -Management of patients with long-term conditions
  • -Management of patients with complex needs
  • -Evidence of appropriate continuing professional development activity to maintain up-to-date knowledge and on-going competence
  • -Evidence of working autonomously and as part of a team
  • -Proven ability to evaluate the safety and effectiveness of their own clinical practice
