Job summary

We are recruiting a Clinical Pharmacist who will relish the challenge of working in primary care. The successful candidate will work with our multi-disciplinary clinical team to support patients.

You will be based at Heartwood Medical Practice, which is part of Swadlincote Primary Care Network (PCN).

You will be part of our supportive and friendly multidisciplinary team and the post holder will provide support to both patients directly and also our internal practice staff with regards to prescription and medication queries.

You will help support the repeat prescribing system, deal with acute prescribing requests, medicine reconciliation and undertake structured medication reviews for safer prescribing.

The post holder will be expected to provide clinical leadership on medicines optimisation and work on Quality Improvement projects.

Main duties of the job

Clinical Pharmacy in General Practice is part of an exciting programme of transformation. Clinical Pharmacy in GP model is supported by the national policy including the GP and Five Year Forward View where there is a need to better utilise the role of pharmacy within primary care to pro-actively help patients stay safe, well and out of hospital as well as helping to reduce the demands on general practice.

The post holder will work alongside peers within Swadlincote PCN as part of a MDT in a patient-facing role. They will support help develop our chronic disease management processes, including undertaking med reviews to proactively manage patients including those with complex polypharmacy.

The post holder will provide primary support to GP staff with regards to prescription and medication queries. They will help support the repeat prescriptions, acute requests, and medicines reconciliation on transfer of care and systems for safer prescribing, providing expertise in clinical medicines advice while addressing both public and social care needs of patients in GP practices.

The post holder will provide clinical leadership on medicines optimisation & quality improvement and manage some aspects of the QOF/Enhanced services. They will ensure that the practices integrate with community and hospital pharmacy to help utilise skill mix, improve patient outcomes, ensure better access to healthcare and help manage workload.

Heartwood Medical Practice is a well-established and highly regarded GP Practice in the centre of Swadlincote.

Our team of almost 40 staff includes 6 GPs, ANPs, Pharmacist, Practice Nurses, Health Coach, Trainee Nurse, HCAs, Social Prescriber and a Reception/admin team.

Heartwood has recently gone through a period of growth, reflected by an ongoing increase in patient list size and team members. To complement our friendly and supportive team, we are seeking a dynamic, enthusiastic and forward-thinking Clinical Pharmacist.

Ideally this role is full time, but open to negotiation for the right candidate. Flexible working and part remote working could be supported.

We are an innovative practice and partnership team, always seeking improvement in how the practice operates and services we offer.

Therefore, we hope to attract an individual who also wishes to take an active role in the future development of General Practice, together with a desire to develop their own clinical interests.
