and caring GP to work between 4-8 sessions per week. A competitive salary is offered to reflect experience.
Heartwood Medical Practice is a well-established and highly regarded GP
Practice, supporting 10,000 patients from our modern premises in the centre of
Swadlincote. CQC rated "Good" at our last review.
We are an innovative practice, continuously seeking improvements in
delivering high quality patient care.

Main duties of the job
Main duties of the job
All GPs have a share of routine consultations, duty days, home visits,
workflow admin and tasks.
Patient consultations are either in-person at the surgery, or remote by
telephone or video. Home visits are shared between GPs and the new Home
Visiting Service.
We are also a Teaching Practice for medical students from the University
of Nottingham and share the mentoring across our GP team.

Job
Purpose
The
GP will manage a caseload and deal with a wide range of health needs in a
primary care setting, ensuring the highest standards of care for all patients
according to their individual needs. They will be part of a multi-disciplinary
team and will be expected to set an example to others, be involved in liaison
with external agencies; assist in education / training and to play a full role
in target achievement and service development.
Clinical
Responsibilities
The
GP will undertake a variety of duties including Practice consultations, both
telephone, video and face to face. Handling patient queries, visiting patients
at home, checking and signing repeat prescriptions and dealing with queries,
paperwork and correspondence in a timely fashion.
- Making
professional, autonomous decisions in relation to presenting problems,
whether self-referred or referred from other health care workers within
the organisation.
- Assessing the health care needs of
patients with undifferentiated and undiagnosed problems.
- Screening patients for disease risk
factors and early signs of illness.
- In consultation with patients and
in line with current Practice disease management protocols, developing
care plans for health.
- Admitting or discharging patients
to and from the caseload and referring to other care providers, as
appropriate.
- Recording clear and contemporaneous
consultation notes to agreed standards, using the clinical computer
system.
- Participating in QOF/IIF related
work.
- Compiling and issuing computer
generated acute and repeat prescriptions.
- Prescribing in accordance with the
Practice prescribing formulary (or generically) whenever this is
clinically appropriate.
- Perform appropriate skilled
examinations with consideration for the patient.
- Respect patients right to
confidentiality and provide information to colleagues
- Respond rapidly to emergencies.
- Ensure up to date emergency
equipment and drugs used if required.
- Ensure up to date with developments
in clinical practice and regularly review knowledge and performance.
