Job summary

Due to increasing list expansion, we are looking for an advanced nurse practitioner / paramedic practitioner / advanced clinical practitioner to join our multidisciplinary, urgent care team.

Main duties of the job

The post holder will apply clinical expertise, levels of judgement, discretion and decision making to the provision of clinical care of patients registered with our practice. We are looking for someone who is able to work autonomously as part of our urgent care team. Consultations are carried out by a mixture of face to face, telephone and home visit appointments, alongside assisting with triaging on the day requests via our online consultation system.

The role will include working primarily alongside GPs and ACPs to meet the needs of our patients, upholding local, national and practice policy and procedures.

MSc in Advanced Clinical Practice (or working towards) and independent prescribing would be desirable but not essential, with the right candidate being encouraged and supported to achieve these qualifications.

About us

We are a high QoF achieving, busy, growing practice, with approximately 17,000 patients. We are a six Partner practice (five GPs & one paramedic practitioner). As a training practice, we are involved in the training of GPs, nurses, ACPs and HCAs and actively support development.

Job description

Job responsibilities

  • Diagnose and manage patient presentations, using advanced clinical skills and judgement, supported by investigations such as blood tests, ultrasound and x-ray.
  • Develop management plans, in conjunction with the patient and their wishes.
  • Prescribe medication when indicated, in line with evidence-based practice and local/national guidelines, reviewing effectiveness and compliance when required.
  • Refer, when appropriate, to secondary care or other services such as district nurses, physiotherapists, and audiology.
  • Communicate and work collaboratively within the general practice team, to meet the needs of patients.
  • Recognise and work within own competence and professional code of conduct, as regulated by the HCPC/NMC
  • Demonstate empathy and compassion when communicating advice to patients and relatives.
  • Support the development of others within the practice.
  • Maintain accurate, up-to-date healthcare records appropriate to the consultation.
  • Encourage patients to take responsibility for managing their own care and treatment, where it is safe to do so, and provide health education.
  • Take a holistic approach to patient care, bringing together their medical, social and psychological needs.

  • Post-registration experience gained by undertaking ongoing personal development and training.
  • Experience of working in primary/urgent care, or equivalent settings.
  • Good understanding of current healthcare issues.

  • Excellent communication and interpersonal skills.
  • Excellent organisation skills including the ability to make decisions and prioritise.
